Endoscopic microsurgical dissection of the esophagus

Abstract: Blunt dissection of the esophagus is considered the least invasive technique in the treatment of either benign or malignant diseases of the esophagus. Its disadvantage is that it has to be carried out blindly. The results may be uncontrollable hemorrhage, unrecognized injuries to the trachea, and damage to the recurrent laryngeal nerve. In order to reduce the degree of invasiveness a new endoscopic microsurgical technique for the dissection of the esophagus has been developed and tried out in animals. This pap… Show more

“…Minimally invasive oesophagectomy (MIO), first described in 1990s [ 14 16 ], has superiority in reducing the risk of postoperative morbidity without compromising oncological outcomes through avoiding thoracotomy and laparotomy [ 4 , 17 20 ]. Theoretically, MIO has an advantage over OE in reduction the risk of IHM to a larger extent.…”

“…[15]. Three different approaches can be used: either an abdominothoracic approach allowing a large mediastinectomy and a lymph node dissection [10-12], a blunt dissection as described by Orringer 113-141, or an endoscopic transcervical dissection as reported by Buess et a/.…”

“…Endoscopic microsurgical dissection (by mediastinoscope) of the oesophagus was described in 1989 by Kipfmuller et al using a specially designed mediastinoscope [45]. The mediastinoscope has a tip that works as tissue dilator and is adapted to run along the shape of the oesophagus to facilitate dissection.…”
