Sus manifestaciones clínicas, especialmente de la variante racemosa (sic), son pleomorfas e inespecíficas, características que hacen de su diagnóstico un desafío para el clínico. El objetivo de este informe fue describir dos casos de neurocisticercosis racemosa (sic), en los cuales las neuroimágenes permitieron hacer el diagnóstico definitivo. El primer caso se trató de una paciente con cefalea persistente y signos neurológicos focales, que requirió múltiples exámenes paraclínicos para, finalmente, llegar al diagnóstico definitivo de neurocisticercosis racemosa (sic) con vasculitis cerebral secundaria. A pesar del tratamiento médico y quirúrgico, la paciente falleció después de diversas complicaciones. El segundo caso se trató de una paciente con antecedentes de neurocisticerosis, que consultó por vómito crónico incoercible y, también requirió múltiples exámenes paraclínicos para concluir que se trataba de un vómito de origen central, secundario a neurocisticercosis racemosa (sic) y obstrucción del cuarto ventrículo. Después del tratamiento médico y quirúrgico, la paciente presentó una leve mejoría. En estos dos casos se evidenció que la adecuada interpretación de las neuroimágenes es fundamental para el diagnóstico de la neurocisticercosis racemosa (sic).

However, there is still some controversy in cases of intraventricular cysts, in which stereotactic guide has also been used with adequate results [40,41]. Yet, I consider that conservative medical therapy is preferable in most cases leaving surgery only for resistant cases, or for patients in whom the diagnosis of NCC cannot be confirmed by imaging and CSF studies or by a therapeutic trial with cysticidal drugs.…”

Neurocysticercosis (NCC) is the most frequent parasitic disease of the human brain. Modern imaging studies, CT and MRI, have defined the diagnosis and characterization of the disease. Through these studies the therapeutic approach for each case may be individualized with the aid of antihelmintics, steroids, symptomatic medicines, or surgery. The use of one or various therapeutic measures largely depends on the peculiar combination of number, location, and biological stage of lesions as well as the degree of inflammatory response to the parasites. Although there is not a typical clinical picture of NCC, epilepsy is the most frequent manifestation of parenchymal NCC, whereas hydrocephalus is the most frequent manifestation of meningeal NCC. Eradication of cysticercosis is an attainable goal by public education and sanitary improvement in endemic areas.
