A four-month-old infant, previously healthy and developmentally normal, presented to the emergency department (ER) of a tertiary care hospital with hematemesis and pallor for one day, as well as a three-week history of irritability and intermittent vomiting. The infant was taken to various hospitals in their town, where he was given symptomatic treatment for vomiting and the mother was advised to feed infant. Nothing out of the ordinary was reported by the parents. The infant's symptoms were managed in the ER, and baseline labs were performed to determine the cause of the blood-stained vomiting and pallor. Except for a low Hb level, the baseline labs were normal. An abdominal x-ray revealed a stainlesssteel blade in his stomach. The ingestion of a blade was unknown to the parents. The case was referred to pediatric gastroenterology for further treatment. The blade was removed through endoscopy, and recovery was uneventful. Conclusion: The importance of supervising infants and young children under all circumstances is emphasized

Endoscopic intervention is reportedly effective in 90% of patients who require retrieval of foreign bodies in the gastrointestinal tract, and this treatment is associated with few serious complications [ 9 ]. Therefore, clinicians should consider endoscopic removal of hazardous foreign bodies in the gastrointestinal tract whenever possible.…”

Background Tobacco ingestion is widely known to cause nicotine toxicity, which may result in severe symptoms. Two heated tobacco sticks, called TEREA™ and SENTIA™, were launched in 2021 by Philip Morris International (New York, NY, USA), and their ingestion is associated with a risk of bowel injury because they contain a partially pointed metallic susceptor. However, this risk is not well known to the general public or healthcare providers. To increase awareness of this risk, we herein report a case involving extraction of a metallic susceptor after ingestion of the heated tobacco stick TEREA™. Case presentation A 7-month-old girl presented to the emergency department of a nearby hospital because she was suspected to have accidentally swallowed heated tobacco. Although she presented with no symptoms related to nicotine poisoning, abdominal X-ray examination revealed a metal object in her stomach. According to a statement released by the Japan Poison Information Center, the TEREA™ heated tobacco stick contains a metallic susceptor with a rectangular shape and sharp corners. The patient was transferred to our department because of the risk of bowel injury, and upper gastrointestinal endoscopy was performed. No cigarettes were found by endoscopic observation; however, a metallic susceptor was located in the second part of the duodenum. We grasped it with biopsy forceps and carefully removed it using an endoscope with a cap attached to the tip. The post-endoscopic course was uneventful. Conclusions Some patients who ingest heated tobacco sticks might be exposed not only to the effects of nicotine but also to physical damage caused by a metallic susceptor. Infants and toddlers especially could swallow these sticks, therefore tobacco companies need to make the problem more public. Clinicians also should alert the problem, and pay attention to this risk in the clinical setting.

Esophageal foreign bodies are common cases in hospitals and if not treated immediately, may lead to serious complications. This study aimed to determine the various methods of esophageal foreign body management that can be performed. This was a literature review study. Data were searched from three databases, namely Clinical Key, PubMed and Google Scholar. The results obtained 11 literatures to be reviewed. In addition to esophagoscopy, there were several other management techniques, including medical management using proteolytic enzymes, glucagon, and nitroglycerin, and other operative management, namely procedures with a dual-channel endoscope, Foley catheter, and double fogarty balloon catheter according to the type and location of the foreign body obstruction. In conclusion, management of esophageal foreign bodies includes both medical and operative management according to the type and location of foreign body obstruction in the esophagus. However, esophagoscopy method is the gold standard for esophageal foreign body management. Keywords: esophageal foreign body; medical management; operative management; esophagoscopy Abstrak: Benda asing esofagus merupakan kasus yang sering ditemukan di rumah sakit yang jika tidak segera ditangani dapat menyebabkan komplikasi serius. Penelitian ini bertujuan untuk mengetahui berbagai metode penatalaksanaan benda asing esofagus yang dapat dilakukan. Jenis penelitian ialah suatu literature review. Pencarian data menggunakan tiga database yaitu Clinical Key, PubMed dan Google Scholar. Hasil penelitian mendapatkan 11 literatur untuk dikaji. Selain esofagoskopi yang sering digunakan, terdapat beberapa teknik penatalaksanaan lain yaitu tatalaksana medis dengan menggunakan enzim proteolitik, glukagon, dan nitrogliserin serta tatalaksana operatif lain yaitu prosedur dengan dual-channel endoscope, Foley catheter dan double Fogarty balloon catheter yang dapat dilakukan sesuai dengan jenis dan lokasi obstruksi dari benda asing. Simpulan penelitian ini ialah penatalaksanaan benda asing esofagus dapat dilakukan dengan tatalaksana medis dan operatif sesuai dengan jenis dan lokasi obstruksi benda asing di esofagus.
