Endoscopic Management for Post-Surgical Complications after Resection of Esophageal Cancer

Abstract: Background: Esophageal cancer (EC) is the sixth-leading cause of cancer-related deaths in the world. Esophagectomy is the most effective treatment for patients without invasion of adjacent organs or distant metastasis. Complications and relevant problems may occur in the early post-operative course or in a delayed fashion. Here, innovative endoscopic techniques for the treatment of postsurgical problems were developed during the past 20 years. Methods: Endoscopic treatment strategies for the following postoper… Show more

“…It can loosen the fibrous scar around the stenosis by an external force, break the muscle fibers at the anastomotic stenosis, and expand it. The esophageal lumen can improve the current symptoms such as dysphagia and eating obstruction [ 20 ]. However, with the deepening of stent implantation, it gradually replaced dilatation and became the preferred method, which can not only achieve the effect of dilation but can also reduce the number of expansions, relieve clinical symptoms, and improve postoperative quality of life [ 21 ].…”

“…Para-anastomotic collections have to be drained either by percutaneous drain or re-operation. The stent may be left in place for six to eight weeks and can be removed endoscopically at the end of therapy [ 8 ].…”

“…One minimally invasive endoscopic treatment approach for this diagnosis is the use of EVT [14]. Several studies have demonstrated high closure rates of approximately 90% with a mortality rate of 10% [18]. In our cohort, all 21 patients with an ECCG type II leak received EVT.…”

“…Using stents is an established therapeutic option, and endoscopic vacuum therapy (EVT) has become a promising alternative [16,17]. Several studies have demonstrated high closure rates of approximately 90% [18]. EVT can be used in two different ways: intraluminal positioning of the sponge in the esophageal lumen and intracavitary placement to treat a para-esophageal wound cavity [19].…”
