Endoscopic endonasal anatomy of the nasopharynx in a cadaver model

Abstract: Surgical access to the nasopharynx has posed significant challenges in the treatment of recurrent or persistent nasopharyngeal carcinoma. This study demonstrated that endoscopic dissection of this region is feasible and has the potential to completely exenterate these lesions.

“…Wen et al suggested novel orientation landmarks for EEN, including first the foramen lacerum laterally, then the eustachian tube isthmus posterosuperiorly, lateral pterygoid palate laterally, and longus capitis and stylopharygeal fascia posteriorly. The fossa of Rosenmuller (FOR) can be a useful landmark; however, the distance varies between the apex of the FOR and the PPICA, and the FOR is frequently compromised when performing EEN. The eustachian tube is also a good landmark, as it is always anterior and medial to the PPICA .…”

“…This exposes the cartilaginous Eustachian tube, which can be removed. The limits of resectability are poorly defined and debatable, but some authors cite the clivus posterosuperiorly, lateral pterygoid plate laterally, inferior turbinate anteriorly, and the level of the soft palate inferiorly . The parapharyngeal ICA is at risk during dissection after removal of the pterygoid plates and is the oncologic limit of dissection.…”

“…In Germany, a new “Theatrum anatomicum,” similar to the ancient anatomical theatre provides an opportunity for learning anatomy and difficult surgical procedures outside the operating room by making use of cadavers [77]. In addition, many medical training centers conduct CD workshops in order to develop new surgical procedures [78, 79]. The institutions conducting specialty-specific training courses on cadavers are shown in Table 3.…”
