Endoscopic electrocauterization of pyriform sinus fistula

Abstract: To determine the efficacy of endoscopic electrocauterization for pyriform sinus fistula (PSF) using a flexible Bugbee cautery electrode. From 2009 to 2016, a total of eight patients with acute suppurative thyroiditis or cervical abscess secondary to PSF were retrospectively registered in our study (three males, five females; median age 6.5 years). All patients underwent endoscopic electrocauterization as treatment for PSF. Six of eight patients had no recurrence after the initial endoscopic electrocauterizatio… Show more

“…CPSF is a rare anomaly from failure of the third and fourth brachial clefts to involute and few studies in the literature have more than a handful of patients. [ 2 , 3 ] There is no significant difference between males and females and 90% of cases occur on the left side. [ 14 ] The asymmetric development of branchial apparatus may be the cause of the left-sided predominance of CPSF.…”

“…The main clinical manifestations are recurrent neck abscess and suppurative thyroiditis that are predominantly left sided. [ 1 2 ] Most clinical literature on CPSF exists in the form of case reports. Large clinical reports and systematic studies are rare.…”

“…However, it is difficult to find the fistula in traditional open-neck surgery, or the resection is incomplete, which may lead to recurrence (1,6). In recent years, minimally invasive endoscopic occlusion of the internal opening for the treatment of PSF is performed, including biocauterization (7), electrocautery (8), chemocautery (9), coblation (10), laser cauterization (11), and suture closure (6), etc. But only a few cases are reported with high recurrence rate.…”

“…Complications and recurrence also occurred. Researchers have increasingly advocated for endoscopic cauterization of the sinus tract opening in the pyriform sinus [17, 2022]. Unfortunately, this method will create a closed cavity.…”
