Objectives Congenital pyriform sinus fistula (CPSF) is a rare disease that can be easily misdiagnosed. This study investigates the value of ultrasonography in the early diagnosis and treatment of CPSF in children. Methods Clinical features and ultrasonography images of 31 CPSF pediatric patients confirmed by operation were retrospectively analyzed, different sonographic features during the infection period and the quiescence period were summarized and the consistency test of ultrasonic recognition and diagnosis between observers was conducted. Results In this study, 25 CPSF children had thick-walled cystic masses during the infection period, and cystic masses of 8 cases showed gas echo inside; after the modified valsalva maneuver, gas echo was found in another 5 cases. The detection rate of gas can be enhanced through the modified valsalva maneuver and infants’ cry so as to provide an important basis for the diagnosis of pyriform sinus fistula. During the quiescent period of inflammation of 6 cases, fistula can be completely shown, and the wall structure has not been completely destroyed, so that the running position of fistula can be clearly seen. Ultrasonography boasted a good inter-observer consistency in identification and determination (Kappa:0.799–0.857; P<0.001). Conclusion Ultrasonography could clearly reveal the position and direction of CPSF fistula. Different ultrasonic characteristics in different periods could provide relevant information for the selection of clinical operation timing and evaluate the post-operative effects.

El estudio del nódulo pulmonar en pediatría es un desafío diagnóstico, se debe tener en cuenta diferentes patologías, especialmente infecciones. En países en desarrollo, donde la infección por tuberculosis es endémica, es uno de los diagnósticos más probables; sin embargo, las neoplasias como posibilidad diagnóstica no deben desestimarse.Objetivo: Describir el caso clínico de una paciente con un nódulo pulmonar en quien después de descartar las causas más frecuentes se diagnosticó una malignidad pulmonar primaria.Caso Clínico: Adolescente de 17 años, consultó por cuadro de un mes de evolución de tos, disnea y hemoptisis sin otros síntomas asociados, sin respuesta a terapias antibióticas habituales, por lo que se realizó una tomografía de tórax contrastada donde se evidenció un nódulo pulmonar en lóbulo inferior derecho de características irregulares, se descartó un proceso infeccioso por estudios de lavado bronco-alveolar (para tuberculosis, hongos y otras bacterias) y se completó estudio con biopsia, que fue compatible con carcinoma mucoepidermoide. Fue operada con lobectomía de lóbulo inferior derecho con vaciamiento ganglionar con respuesta clínica adecuada a tres años de seguimiento.Conclusión: El nódulo pulmonar en pediatría es un hallazgo que se asocia especialmente a infección, sin embargo, se debe tener en cuenta condiciones neoplásicas, no solo metástasis sino también lesiones malignas primarias del pulmón por las implicaciones en el pronóstico.

Introduction Abnormalities of the fourth branchial arch are less common than those of the second arch and usually present with inflammation of the left thyroid lobe. Case presentation We report the case of a 10 years old girl who presented to our department with recurrent cervical cellulitis, and who was diagnosed, upon endoscopic exploration, with a left sinus pyriform fistula. The patient was treated using mini-invasive surgery by electrocoagulation, with good clinical outcome. Conclusion Branchial arch malformations are rare congenital malformations. The diagnosis is mainly based on clinical examination, imaging and, endoscopic investigations. The conservative attitude may be the treatment of choice, especially if the cervical mass is not well individualized.
