Endoscopic CO2 Laser Management of Laryngocele

Abstract: CO2 laser-assisted endoscopic excision of a laryngocele is a quick, precise, and safe alternative to an external approach excision (lateral thyroidotomy, laryngofissure) with fewer complications than its external counterparts, resulting in speedier rehabilitation of both the patient and his or her voice.

“…Other authors use the term laryngocele regardless of the contents of the lesion. 3,4 The use of the term "laryngocele" in reference to patients who have undergone supraglottic laryngectomy is somewhat problematic, in that the upper surface of the laryngeal ventricle has been surgically removed. Thus, these lesions do not form in the same manner as most laryngoceles.…”

“…2 Surgical drainage and marsupialization are the treatments of choice for laryngoceles, either from an external approach or by endoscopic laser excision. 3,4 Most laryngoceles are unilateral and, thus, do not threaten the patient's airway. However, laryngoceles may arise bilaterally in the posttreatment setting and may be a life-threatening complication of therapy.…”

“…Computed tomography is the most useful ancillary investigation in cases with suspicion of concomitant laryngeal pathology. It provides a defi nitive diagnosis of a laryngocoele through cross sectional imaging and contrast resolution superior to that of plain radiography [9]. Magnetic resonance imaging, which provides high defi nition of soft tissues, offers detailed information on the boundaries of the air-fi lled sac and, in particular, on its relation to the thyrohyoid membrane, distinguishing the internal from the external or the mixed components of this cyst.…”

“…The cervical approach employed for treatment of external laryngoceles can disrupt the normal function of the strap muscles that are important for tone generation and therefore surgery in asymptomatic young musicians is to be deferred, as it would hamper the performer's musical career [15]. Surgical excision by means of LASER has been employed in centers where Laser equipment is available [9]. We performed bilateral excision of the laryngoceles via trans-cervical approach in the professional wind instrument player and the manual laborer both of who were symptomatic.…”

“…Endolaryngeal approach by means of endoscopic CO 2 laser can be used for internal laryngocoeles. 15 For combined laryngocoeles external approach is indicated which may include one of the following-(a) Transthyrohyoid membrane approach, (b) thyrotomy for larger ones, with resection of upper 1/3 of thyroid cartilage, (c) V thyrotomy. 16 We employed external approach via thyro-hyoid membrane.…”
