Background: To investigate whether endoscopic stapes surgery is safer and less invasive than conventional stapes surgery using an operating microscope.Methods: The subjects were 60 patients who were divided consecutively into group A undergoing microscopic stapedotomy and group B undergoing endoscopic stapedotomy. The procedures for endoscopic surgery were fundamentally the same as those for microscopic surgery, except for post aural incision used in some patients in microscopic group. The two surgical techniques were compared with respect to the operating time, approach, drilling of posterosuperior canal, manipulation of chorda tympani, visualisation of anterior crus, postoperative hearing, postoperative pain, and complications. Results: There were no differences of operating time or postoperative hearing between the endoscopic and microscopic groups. There was very little postoperative pain in the endoscopic group. Drilling at the posterosuperior part of the external auditory canal was less extensive in the endoscopic group than in the microscopic group. Visualisation of anterior crus of stapes was good in endoscopic group.Conclusions: Endoscopic stapes surgeries are technically feasible, safe and promising. The main advantages were: easy access, virtually no trauma to the chorda tympani nerve and excellent vision. The disadvantages were the lack of stereoscopic vision, having to work with one hand only and the learning curve.

St apedotomies are perfomed with the aid of surgical microscopes. However, the microscope has some limitations and may cause complications such as damage to the chorda tympani nerve. There are just a few cases and no series published on the use of sino-nasal endoscopes in stapedotomies.Objectives: a) To investigate the feasibility of using sino-nasal endoscopes in stapedotomies, focusing on the visualization of important structures. b) To show initial results, discuss possible advantages and disadvantages of this instrument.Methods: 15 patients with otosclerosis were selected to undergo stapedotomies in 2010. The data and surgery videos were analyzed retrospectively (study case series). The anatomical conditions of the oval window niche and surgical steps were described and used to assess possible benefits of such procedures. Results:The surgeries were performed with endoscopes only and all relevant anatomical structures were visualized without difficulty. No complications were observed and 14 of the 15 patients reported improvement of their hearing, confirmed by postoperative audiological tests. Conclusion:Totally endoscopic stapes surgeries are technically feasible, safe and promising. In this small series, the main advantages were: virtually no trauma to the chorda tympani nerve and excellent vision. Minimally invasive, in particular endoscopic treatment modalities are an important topic in medicine today. Over the last 15 years mini-endoscopy (ME) has been introduced and established in different fields of the head and neck region. ME includes endoscopy of the Eustachian tube, middle ear, lacrimal duct system and duct system of the major salivary glands. Direct visualization by endoscopy enables rapid, efficient and accurate diagnosis and treatment planning according to the findings. In the majority of cases simultaneous interventional therapy is possible. This has lead to a fundamental change in treatment regimes which now aim to preserve the anatomic structure and function. In specialized centers the frequency of external dacryocystostomy could be reduced to less than 10% by means of endoscopic surgery of the lacrimal system, while the frequency of parotidectomy was reduced to below 5%. This development allows not only for medical advances, but also for the desired cost reduction on the part of the administrative organs responsible for providing medical care.
