Endoscope‐assisted resection of intravestibular Schwannoma: A video case report

Abstract: Intravestibular schwannomas are a rare cause of unilateral sensorineural hearing loss. Management of these tumors involves translabyrinthine resection, for which complete visualization around the angles of the vestibule may be limited under an operating microscope. We present the first reported case of an endoscope‐assisted resection of an intravestibular schwannoma, along with the operative video recording. Using additional information gained from endoscopic examination of the tumor and its resection site, we… Show more

“…Endoscopes may decrease the need for dissection of normal structures for the sake of exposure and enhance quality of visualization around deep angles and recesses. 4,19 While commonly used for resection of cholesteatoma and tympanoplasty, the endoscope has also been successfully utilized to resect ILS and ensure macroscopic clearance of disease. 4,8,12,20 Pan and colleagues 4 reported an endoscopic-assisted resection of an ILS in 2019.…”

“…“Vestibulocochlear ILS” is another type of ILS that affects the cochlea and vestibule ± semicircular canals contribution. The last type in this classification is “transotic ± cerebellopontine angle” (tumor in cochlea and/or in the vestibular portion of the inner ear; extension into the middle ear and into the IAC with or without extension to CPA) 1,2. The symptoms of ILS are similar to other cochleovestibular disorders and are not specific to the disease.…”

“…The presentation of intravestibular schwannomas is unilateral sensorineural hearing loss and the vestibular symptoms are most often observed in patients 2. Schwannomas associated with these symptoms usually exhibit gadolinium contrast enhancement on T1-weighted images and a “filling defect” on T2 or CISS images 1,2…”
