Endoscope-assisted contralateral transmaxillary approach to the clivus and the hypoglossal canal: technical case report

Abstract: Clival lesions are still considered surgically complex due to their anatomical location. Critical structures, such as the internal carotid arteries (ICAs), cavernous sinuses, cranial nerves, and brainstem, may be encased within the lesion. Although advances in endoscopic endonasal approaches have provided new routes to these lesions, exposure and resection of clival tumors through the endonasal route remain a technical challenge. Here, the authors report a left-sided endoscopic transmaxillary approach to acces… Show more

“…9,11 For such cases, CTM could be a good surgical alternative or adjunct. [13][14][15] The CTM presents significant advantages over a classical EEA for lesions in the coronal plane, especially if intimately related to the ICA. Borghei-Razavi et al have discussed the limitations of an ipsilateral EEA to the PA. 8 The single most important benefit provided by CTM is the "angle advantage."…”

“…Its major limitation is that it is a cadaver dissection-based study, not performed in the clinical setting. Although the clinical feasibility of CTM has already been demonstrated [13][14][15] ; further studies are necessary to prove its superiority in terms of surgical and clinical outcomes.…”

“…Per this review, the only other reports of this approach come from a case series and 2 case reports, with 3 of the 4 publications coming from the same institution. 8,15,16 In this article, we describe the use of the CTM approach to the petrous apex in a tertiary academic center with minimal prior experience in expanded endoscopic approaches to the skull base. This report adds to the sparse literature examining this complex approach and attests to the reproducibility of prior results.…”

“…The average age was 51.6 years (range = 19-78 years) with a male:female ratio of 15:14. Diagnoses confirmed pathologically included chondrosarcoma (12), chordoma (11), meningioma (two), schwannoma one metastasis (one), petrous apicitis (one), and arachnoid cyst (one). Eleven patients (50% of chondroid neoplasms) presented with recurrent tumors following surgery/radiation therapy: chordoma (nine) and chondrosarcoma (two).…”
