Background: Endosalpingiosis is assumed to be the second most common benign peritoneal pathology after endometriosis in women. Although recent studies indicate a significant association with gynecologic malignancies, many underlying principles remain unclear. This work aimed to systematically describe the intraoperative appearance of endosalpingiosis. Methods: Data and intraoperative videos of patients with histologically verified endosalpingiosis were retrospectively reviewed. The main outcome measures were macroscopic phenotype and anatomical distribution. Additionally, a systematic review searching PubMed (Medline) and Embase was conducted. Results: In the study population (n = 77, mean age 40.2 years (SD 16.4)), the mean size of lesions was 3.6 mm and the main visual pattern was vesicular (62%). The most frequent localization was the sacrouterine ligaments (24.7%). In the systematic review population (n = 1174 (210 included studies overall), mean age 45.7 years (SD 14.4)), there were 99 patients in 90 different studies with adequate data to assess the appearance of the lesions. The mean size of the lesions was 48.5 mm, mainly with a cystic visual pattern (49.5%). The majority of the lesions affected the ovaries (23.2%), fallopian tubes (20.4%), or lymph nodes (18.5%). Comparing this study to the literature population, the main differences concerned the size (p < 0.001) and main visual patterns (p < 0.001) of lesions. Conclusions: The usual intraoperative findings of endosalpingiosis appeared less impressive than described in the literature. In our study population, lesions of a few millimeters in size with a vesicular appearance were mostly seen, most frequently in the sacrouterine ligament area. Intraoperative recognition by the gynecologic surgeon and histologic diagnosis should play an important role in further understanding this entity, scientifically and clinically.

Tujuan: melaporkan kasus endosalpingiosis pada wanita, 36 tahun, dengan keluhan nyeri perut bagian bawah yang dialami sejak 4 tahun terakhir. Kasus: Wanita, 36 tahun, P1A0, datang ke poliklinik dengan keluhan nyeri perut bagian bawah dialami sejak 4 tahun terakhir. Haid teratur tiap bulan dengan lama haid 4-5 hari disertai dismenorea. Riwayat keputihan dan demam disangkal. Riwayat persalinan secara pervaginam tahun 2004 dan tidak pernah menggunakan alat kontrasepsi. Tatalaksana Kasus: Teraba massa kistik pada adneksa kanan, mobile, terasa nyeri tekan. USG ginekologi trans-vaginal menunjukkan massa hipoechoic pada adneksa kanan dan kiri. Laparaskopi menunjukkan uterus ukuran 7x5 cm dengan permukaan licin mengalami perlengketan dengan rektum di bagian posterior, dilakukan adhesiolisis. Terdapat kista paratubal kanan dan pseudocyst pada adneksa kiri, permukaan licin, berisi cairan putih kekuningan. Dilakukan kistektomi pada kedua kista tersebut. Gambaran histopatologi menujukkan cystadenoma serosum dan endosalpingiosis. Post operasi hari ke-10 pasien datang kontrol dengan luka operasi baik dan keluhan nyeri perut berkurang. Simpulan: Follow-up rutin pada kasus endosalpingiosis tidak diperlukan selama tidak ada keluhan yang bermakna dari pasien, karena kasus endosalpingiosis belum pernah dilaporkan akan mengalami transformasi menjadi suatu keganasan. Pasien ini kami ditindaklanjuti 3 bulan dan 6 bulan pertama post operasi, dengan hasil keluhan nyeri perut sudah tidak dirasakan lagi.Kata kunci: endosalpingiosis, nyeri perut bagian bawah, tuba fallopi, sel epithelium Objective: to report endosalpingiosis in women, 36 years old, with complaints of lower abdominal pain experienced since the last 4 years. Case Report: Female, 36 years old, P1A0, came to the clinic with complaints of lower abdominal pain experienced since the last 4 years. Menstruation was reguar each month for 4-5 days with dysmenorrhoea. History of vaginal discharge and fever were denied. The patient had history of vaginal delivery in 2004 and never used contraceptives. Case Management: Cystic mass in the right adnexal, was palpable, mobile, and tender. Gynecologic trans-vaginal ultrasound showed hipoechoic mass on the right and left adnexa. Laparoscopy showed uterine size 7x5 cm with slippery surfaces, having rectal adhesions in posterior part, subjected, therefore, to adhesiolisis. There was a right paratubal cyst and pseudocyst on left adnexa, slippery surfaces, containing a yellowish white liquid. Cystectomy was performed on both cysts. Histopathologic features showed cystadenoma serosum and endosalpingiosis. Day 10 postoperative the patient made a visit, surgical wound was improved and complaints of abdominal pain decreased. Conclusion: Routine follow-up in endosalpingiosis cases is not necessary as long as there is no significant complaints from patients, because endosalpingiosis case has never been reported as undergoing transformation into malignancy.
