Endometrial morphology and pituitary-gonadal axis dysfunction in women of reproductive age undergoing chronic haemodialysis--a multicentre study

Abstract: Pathological endometrium morphology is very common in uraemic women of reproductive age undergoing haemodialysis, with proliferative changes in one-third and atrophia in almost a quarter of them. The results of the study suggest a preserved normal reactivity of endometrium on circulating oestrogens.

“…Additionally, lack of cyclic LH release leads to the absence of the LH surge necessary for ovulation [3]. Estradiol and progesterone are decreased in uremic women, which can result in uterine atrophy [4]. Reduced prolactin clearance by the kidneys also results in hyperprolactinemia, further inhibiting ovulation [5].…”

“…There is an earlier onset of menopause in women with CKD on an average by 4.5 years as compared to general population [6, 7]. Among women on hemodialysis, 73% have menstrual disorders and amenorrhea present in half of them [8]. Women with end-stage renal disease (ESRD), especially with amenorrhea, have high serum prolactin due to impaired renal clearance, increased luteinizing hormone (LH) and follicular stimulating hormone (FSH), and reduced estradiol and progesterone concentration.…”

“…Üreme çağındaki hemodiyaliz tedavisi alan kadınlarda biyopsi ile "patolojik endometrium morfolojisi"nin çok yaygın olduğu görülmüştür (%80) (19). Hemodiyaliz süresi ve/veya sıklığının uzatılması ile hastaların yaşam süresinde artış yanında yaşam kalitesi, fiziksel ve kognitif fonksiyonlarında iyileşmeler gözlendiği bilinmektedir (20)(21)(22).…”
