Effects of Ultramicronized Palmitoylethanolamide (um-PEA) in COVID-19 Early Stages: A Case–Control Study

Ultramicronized palmitoylethanolamide (um-PEA), a compound with antioxidant, anti-inflammatory and neuroprotective properties, appears to be a potential adjuvant treatment for early stages of Coronavirus disease 2019 (COVID-19). In our study, we enrolled 90 patients with confirmed diagnosis of COVID-19 that were randomized into two groups, homogeneous for age, gender and BMI. The first group received oral supplementation based on um-PEA at a dose of 1800 mg/day for a total of 28 days; the second group was the control group (R.S. 73.20). At baseline (T0) and after 28 days of um-PEA treatment (T1), we monitored: routine laboratory parameters, inflammatory and oxidative stress (OS) biomarkers, lymphocytes subpopulation and COVID-19 serological response. At T1, the um-PEA-treated group presented a significant reduction in inflammation compared to the control group (CRP p = 0.007; IL-6 p = 0.0001; neutrophils to lymphocytes ratio p = 0.044). At T1, the controls showed a significant increase in OS compared to the treated group (FORT p = 0.05). At T1, the um-PEA group exhibited a significant decrease in D-dimer levels (p = 0.0001) and higher levels of IgG against SARS-CoV-2 (p = 0.0001) compared to the controls. Our data demonstrated, in a randomized clinical trial, the beneficial effects of um-PEA in both asymptomatic and mild-symptomatic patients related to reductions in inflammatory state, OS and coagulative cascade alterations.

OBJECTIVE: Compression/thinning of the endometrial thickness with the effect of progesterone during the luteal phase of the menstrual cycle is defined as endometrial compaction. This study aimed to show the effect of changes in endometrial thickness in the luteal phase of intrauterine insemination cycles on pregnancy outcomes. STUDY DESIGN: Fifty-eight patients who were planned for intrauterine insemination were included in this prospective cohort study. The effect of the change between the patients' endometrial measurements on the day of trigger and the 7th day after intrauterine insemination on pregnancy outcomes were compared. Patients were divided into 3 groups according to endometrial thickness change. Those with an endometrial thickness change of less than 5% were called the no change group (n=18). The groups with endometrial thickness decreasing more than 5% (n=9) and increasing (n=31) formed the other two groups. In addition, the effect of luteal phase support on endometrial compaction was investigated. RESULTS: The highest pregnancy rate (55.6%) was observed in the group with more than 5% endometrial thinning. It was found to be 16.7% in the group without change and 19.4% in the group with an increased endometrial thickness (p=0.045). endometrial compaction and pregnancy rates were higher in the group given oral dydrogesterone for luteal support compared to those using vaginal micronized progesterone. CONCLUSION: Endometrial compaction increases clinical pregnancy in intrauterine insemination cycles. Further studies are necessary to confirm the results of this study.

Objective To assess the impact of an estrogen replacement regimen on frozen embryo transfer (FET) cycle outcome. Methods In the present retrospective cohort study, data of infertile women undergoing FET with good‐quality embryos were reviewed. The first group received 2 mg of estradiol hemihydrate (EH) once daily for 6 to 7 days, then twice daily for 4 to 5 days, and then three times a day until embryo transfer. The second group received EH twice daily for 7 to 8 days, then three times a day. The third group received EH three times a day constantly. The main outcome measure was live birth rate. Results In total, 394 FET cycles were included. The fixed 6‐mg group required the highest estradiol hemihydrate dose. The duration of estrogen treatment was significantly longer in the first group. Maximal endometrial thickness was highest in the second group (10.2 ± 1.3 mm vs. 9.6 ± 1.4 mm vs. 8.6 ± 0.9 mm, respectively; P < 0.001). The clinical pregnancy rates in the groups were 41.1%, 55.2%, and 42.2%, respectively (P = 0.035). Live birth rates were 40.8%, 50.9%, and 48.1%, respectively (P = 0.320). Conclusion In FET cycles with hormone replacement therapy, a step‐up 4‐mg regimen provides thicker endometrium with optimal‐dose estrogen in an adequate time frame. The tendency for increased clinical pregnancy and live birth rates is remarkable.
