Endolymphatic volume in patients with meniere's disease and healthy controls: Three‐dimensional analysis with magnetic resonance imaging

Abstract: ObjectiveRecently, 3‐Tesla magnetic resonance imaging (MRI) with intravenous gadolinium injection has been used to reveal endolymphatic hydrops (EH). In the present study, we aimed to evaluate EH in patients with Meniere's disease (MD) objectively and quantitatively, and compared the endolymphatic space (ELS) in individuals with MD and healthy controls, to gain understanding of the characteristics of MD.MethodsEighty‐two patients with unilateral MD (uMD), 16 patients with bilateral MD (bMD), and 47 healthy vol… Show more

“…However, these 3D analyses are time intensive and impractical for routine work. Therefore, we proposed a convenient semi-quantification volume-referencing grading system according to the reported inner ear fluid volume results ( 9 , 10 ). The VR scores showed a better correlation with the MD clinical features than the Bernaerts scores described by Bernaerts et al ( 13 ).…”

“…The radiologists were blinded to the patient's information according to an adjusted 2D volume-referencing semi-quantification grading system (VR scores, Figure 1 ). We preferred 2D semi-quantification analysis and made an adjusted criteria according to the inner ear fluid volume ( 21 ) and the normal endolymph ratio ( 10 ) described by Inui et al and Ito et al ( Table 1 ). In brief, the degree of ELH in the vestibule and cochlea was assessed by visual comparison of the relative areas of the non-enhanced endolymphatic space vs. the contrast-enhanced perilymph space in several axial planes.…”

“…Following this, patients received additional examinations, including vestibular-evoked potential tests, the video head impulse test, electrocochleography and MRI to show presence of endolymphatic hydrops. 4 A diagnosis was reached according to the classification of vestibular disorders from the Barany Society [5][6][7][8][9][10][11] and Japan Society for Equilibrium Research in 1987 and 2017 (in Japanese). In addition, orthostatic dysregulation was diagnosed according to the criteria defined by Joseph et al, 12 and psychogenic dizziness was diagnosed according to the criteria by Dieterich et al 13 and by consultation with specialised psychiatrists.…”
