Endoillumination (chandelier) assisted scleral buckling for a complex case of retinal detachment

Abstract: Endoilluminator-assisted scleral buckling combines the advantages of scleral buckling for its external approach and pars plana vitrectomy for its better visual visualization in the management of retinal detachment (RD). It has recently been proven to be safe and efficacious in simple cases. This report discusses successful management of a complex case of RD in a patient with the single functioning eye, where vitrectomy was expected to have a complicated course.

“…A 360° peritomy and extraocular muscle bridling was followed by placement of 25 gauge self-retaining endoilluminator either in a quadrant opposite to the suspected break or else in inferotemporal quadrant. Under high microscopic magnification and through volk Reinverting Operating Lens Systemand wide angle viewing system (WAVS), break was localised using indentation, marked and cryopexy was performed[ 2 ] [ Fig. 1 ].…”

“…There are three procedures commonly used for repair of primary rhegmatogenous retinal detachment: conventional buckling, pneumatic retinopexy, pars plana vitrectomy or a combination of these procedures. 1,2 With advances in surgical techniques and instrumentation, vitrectomy is being used more frequently in the initial management of retinal detachments.…”

“…Using a chandelier during scleral buckling helps us to visualize the retina better with better magnification and illumination. [10] These advantages of chandelier-assisted scleral buckle helped us to locate the break over the foreign body.…”
