Endogenous glycosides in critically ill patients

Abstract: Different types of endogenous glycosides including endogenous ouabain are elevated in a significant proportion of critically ill patients. The occurrence of these substances is associated with increased morbidity and hospital mortality rates, possibly due to systemic inflammatory processes. DLIS but not endogenous ouabain concentrations were found to be related to left ventricular function.

“…Other polar cardiotonic steroids of unknown structure are present in bovine tissue [25] and human plasma (Hamlyn et al, unpublished observation). (3) The immunoassay format, that is, radioimmunoassay [37,45,53,[56][57][58]62,[65][66][67][68][69]76,79,100] vs. ELISA [7,23,46,60,61,64,75,77,98], EIA [59,99] or DELFIA [63] used. Different formats have certain advantages but also tend to report method-specific values and differ in susceptibility to interferants.…”

“…By itself, the observation of a J-shaped relation of endogenous ouabain with salt balance is interesting but hardly noteworthy. However, in virtually all clinical studies in which endogenous ouabain is chronically elevated, outcome is universally poor [15,56,57 ■■ ]. This raises the possibility that high endogenous ouabain per se either is a biomarker and/or a functional mediator of adverse cardiovascular outcomes.…”

“…We have suggested that digoxin should not be given to patients in whom the ambient concentrations of endogenous CGs are increased (15 ). This highlights a need for the development of analytical methods that use clinically relevant volumes of blood for the routine determination of endogenous CGs so that the appropriate information concerning optimal therapy can be obtained (12 ).…”
