Endogenous Candida endophthalmitis with no apparent predisposing factors

Abstract: particularly in the right eye. Two months following laser photocoagulation and 6 weeks after commencement of warfarin, the disc new vessels had regressed. Six months into follow-up, his condition is stable. CommeJltAntiphospholipid antibodies were first detected as a result of disturbances they cause in routine biological tests. These effects include lupus anticoagulant in the augmented partial thromboplastin test (APTT) and false serological tests for syphilis in the VDRL test. The systemic clinical features … Show more

“…The culture for the same strain of Candida was positive for the nail and the eye. The second case had used inhaled steroids for asthma and could have developed asymptomatic oropharyngeal candidiasis [6]. From these reported cases and the experience in our case, it appears that endogenous candida endophthalmitis could occur from mild fungal infections from a distant site such as the toenail.…”

“…Although our patient had a very typical fundus appearance suggestive of fungal endophthalmitis, it was very unusual in that he was immunocompetent and had no apparent risk factors. In the world literature, there are only two case reports of endogenous candida endophthalmitis with no previous predisposing risk factors [5, 6]. The first case was a patient with onychomycosis and vaginal candidiasis [5].…”

“…The great majority of patients are infected parenterally1 and more than half of patients are immunocompromised 11. Reports of endogenous Candida endophthalmitis in patients with no apparent predisposing risk are anecdotal,12 but one case was reported arising soon after skin tattooing in a patient with asplenia 13. Outcomes are often poor with one-third of patients developing severe visual loss (Snellen worse than 6/60) 14…”

“…Thus, clinical diagnosis of this condition may become challenging in patients with no underlying factors or those who present with atypical ocular signs. Misdiagnosis of the condition or delay in initiation of treatment may lead to devastating visual outcome [14]. So, diagnostic vitreous tap or diagnostic vitrectomy is recommended for definitive diagnosis in suspicious conditions.…”
