Endodontic treatment of maxillary lateral incisors with anatomical variations

Abstract: Maxillary lateral incisors usually exhibit a single root with a single canal. However, maxillary lateral incisor teeth with unusual morphology of root canal system are frequently reported. These cases of variable root canal anatomy can be treated well by nonsurgical endodontic methods. A detailed description of root canal morphology is fundamental for successful endodontic treatment. Treatment using an operating microscope, radiographs from different angles, and cone-beam computerized tomography (CBCT) can pro… Show more

“…CBCT overcomes these limitations by enabling us to visualize the third dimension and also eliminating super-impositions. It also aids in-depth understanding of the true morphology of root canals 26,27. CBCT in the present case guides the assessment of severity of the curvature of the root canals, the number of roots and root canals, and identifying separated or unseparated root canals.…”

“…However, cases of maxillary lateral incisors with more than one canal and separate roots have been reported [4, 8, 27]. Variations in the number of canals are associated with dental anomalies or intrusive trauma to the primary teeth during the development of the permanent successors [27, 28].…”

“…Maxillary lateral incisors normally have a single root and single canal [3]. However, morphological variations for these teeth include the presence of two [4, 5], three [6, 7], four [8], and even five canals [9], usually associated with the occurrence of traumatic stimuli during tooth development process [10]. Other morphological variations such as dens invagination [9, 11], radicular groove [12], and fusion [13] have also been reported.…”

“…One such precaution was the staining of the tooth with methylene blue which, theoretically, would enhance the visibility by showing how far the PRG extend into dentin. 30 Recent evidence, however, suggests that staining may not necessarily enhance the detection of cracks because the dye cannot flow into craze lines unless there is a break on the surface. Once a crack propagates into a fissure or fracture, these can be stained with dyes.…”

“…Therefore, clinicians must be aware of the many variations in the root canal morphology of the tooth. 7 , 30 The success of the technique we implemented rests on proper infection elimination, adequate bone formation, disappearance of the underlying pocket, regaining of form and function, and patient's comfort.…”
