Sevoflurane and isoflurane are among the most commonly used general anesthetics for children including infants, but their impact on metabolism, especially on blood glucose level, in children is not well understood. We investigated the impacts of anesthesia of neonatal (7-8 days old) and adult (2-3 months old) mice with the inhalational anesthetics 2.5% sevoflurane or 1.5% isoflurane, or the injectable anesthetics propofol (150 mg/kg) or avertin (375 mg/kg), for up to 6 hours. We found that sevoflurane and isoflurane induced severe hypoglycemia in neonatal mice and that this phenomenon was specific to the inhalational anesthetics because the injectable anesthetics propofol and avertin did not induce hypoglycemia. Surprisingly, the inhalational anesthesia induced hyperglycemia instead in adult mice. We also demonstrated that the inhalational anesthesia-induced hypoglycemia was a major cause of death for the neonatal mice receiving intranasal administration of saline prior to anesthesia. These studies revealed severe hypoglycemia in neonatal mice during anesthesia with sevoflurane or isoflurane. If this phenomenon also occurs in human, our findings would warrant closely monitoring blood glucose level and maintaining it in the normal range in infants receiving inhalational anesthesia.

ΠΕΡΙΛΗΨΗ.Στη σύγχρονη κτηνιατρική πράξη, ο κτηνίατρος που ασχολείται με την αναισθησία αντιμετωπίζει πολλές φορές το δίλημμα της επιλογής του καταλληλότερου και ασφαλέστερου αναισθητικού σχήματος. Παράλληλα, οι αλματώδεις εξελίξεις της αναισθησιολογίας εξοπλίζουν συνεχώς τον αναισθησιολόγο κτη νίατρο με καινούργια σκευάσματα και νέες τεχνικές, καθιστώντας την τελική επιλογή ακόμη δυσκολότερη. Το σεβοφλουράνιο είναι ένα σχετικά νέο πτητικό αναισθητικό, το οποίο χρησιμοποιείται πλέον ευρέως για την αναισθησία του ανθρώπου και όλο και πε ρισσότερο για την αναισθησία των ζώων. Πρόκειται για ένα φθο-ριωμένο παράγωγο του μεθυλ-ισοπροπυλ-αιθέρα, μη εύφλεκτο, μη εκρηκτικό, άχρωμο, πτητικό υγρό, με ευχάριστη οσμή. Σκοπός της παρούσας ανασκόπησης είναι η περιγραφή των κυριότερων ιδιο τήτων του σεβοφλουρανίου και της χρήσης του στην κτηνιατρική πράξη. Αρχικά αναφέρονται τα κυριότερα φυσικοχημικά χαρα κτηριστικά του σεβοφλουρανίου και η κλινική τους σημασία, η φαρμακοκινητική και η φαρμακοδυναμική του δράση, καθώς και ο μεταβολισμός και η πιθανή τοξικότητα του. Επίσης, συγκρίνεται το σεβοφλουράνιο με τα συνηθέστερα χρησιμοποιούμενα πτητικά αναισθητικά. Στη συνέχεια, περιγράφεται η χρήση του σεβο φλουρανίου στην αναισθησία των συχνότερων στην κτηνιατρική πράξη ζώων συντροφιάς, παραγωγικών ζώων και ζώων εργα στηρίου, καθώς και οι διάφορες ιδιαιτερότητες που εμφανίζονται κατά περίπτωση. Επίσης, περιγράφονται και κάποια ενδεικτικά παραδείγματα χορήγησης σεβοφλουρανίου σε συνδυασμό με άλ λες προαναισθητικές ή αναισθητικές ουσίες, που χρησιμοποιούνται στην κτηνιατρική πράξη. ABSTRACT. In clinical practice die anaesthetist often faces die dilemma of choosing die best fitting agem from a wide variety of injectable and volatile anaesdietic drugs. Meanwhile, die continuous progress of anaesthesiology offers new drugs and modern Techniques, making die final choice even more difficult. Sevoflurane is a new volatile anaesthetic agent, which is nowadays increasingly used in veterinary anaesthesia. It is a fluorinated derivative of methyl-isopropyl-ether. At room temperature it is a non-flammable, non-explosive, colourless liquid, with a pleasant smell. The aim of this review article is to present the basic attributes of sevoflurane and its use in veterinary practice. Its most important physical and chemical properties and their clinical implications, as well as its pharmacokinetics, pharmacodynamics, metabolism and potential toxicity are presented. Furthermore, sevoflurane is compared with other commonly used volatile anaesthetic agents and its use in anaesthesia of various companion, large and laboratory animals is discussed. Examples of sevoflurane administration, and of possible combinations with other drugs used in veterinary anaesthesia, are also given. Haemodynamic changes and stress responses of piglets to surgery during total intravenous anaesthesia with propofol and fentanyl

The purpose of the study was to assess the haemodynamic (blood pressure and heart rate) changes and stress responses (serum cortisol and serum amyloid A [SAA] concentrations) to surgery in piglets during total intravenous anaesthesia (TIVA) with propofol and fentanyl. After preanaesthetic medication with intramuscular midazolam (0.5 mg/kg body mass), ketamine (10 mg/kg) and butorphanol (0.5 mg/kg) anaesthesia was induced in five piglets, with intravenous propofol (1 mg/kg) followed by tracheal intubation and mechanical lung ventilation. Soft tissue surgery was performed in the jugular and inguinal regions during TIVA with propofol (8 mg/kg/h) and fentanyl (35 mg/kg/h). Anaesthesia was maintained for 300 min after surgery as the piglets were the control group of a project involving extracorporeal membrane oxygenation. Mean plasma cortisol concentration decreased significantly (P , 0.05) from 59 + 39.9 nmol/L (mean + 1 SD) before surgery to 7.5 + 2.5 nmol/L 300 min after end of surgical procedure. The mean SAA concentrations increased over the same period from 1.6 + 2.3 mg/mL to 4.2 + 5.6 mg/mL without statistical significance. The baseline ( presurgery) mean arterial pressure (MAP) was 72 + 9 mmHg compared with 72 + 11 mmHg 300 min after end of surgery. Neither heart rate nor lactate concentrations changed significantly over the same time points: heart rate was 104 + 11 and 103 + 15 beats/min whereas mean lactate concentrations were reduced from 1.14 + 0.45 mmol/L to 0.90 + 0.22 mmol/L. Haemodynamic stability, a decrease in serum cortisol and a non-statistically significant rise in mean SAA concentrations suggest that the anaesthetic described suppresses the stress response of piglets to surgery without adverse cardiovascular effects. Therefore, it may prove useful in cardiovascular research.
