Endochondral Pseudocyst of the Auricle in Chinese

Abstract: \s=b\ Thirty-one patients with endochondral pseudocysts of the auricle were seen over a four-year period in the Department of Surgery at the University of Hong Kong. The lesions chiefly affected Chinese males (90.3%); 80.6% involved the scaphoid fossa on the anterior surface of the pinna. They were usually asymptomatic and static in size. Excision of the anterior wall, followed by contour pressure dressing, gave cosmetically satisfactory results in 90% of the patients, and there were no recurrences. Pathologic… Show more

“…It was argued that chronic trauma induced cartilage degeneration due to release of the lysozymal enzymes with progressive dilatation and cyst formation within the auricular cartilage. We strongly support this hypothesis as trauma was seen in most of our cases but this fact was, however, not supported by a study, which disproved any increase in the lysozymal activity in the analysis of aspirated fluid [6]. The second theory hypothesized that congenital embryonic dysplasia of the auricular cartilage as the predisposing factor in the development of pseudocyst.…”

“…The volume of the aspirates was reported to have a range between 0.5 and 10 ml [4,6,18] and we found it between 1 and 3.5 ml.…”

“…As the etiology is obscure, various treatments are followed such as needle aspiration, curettage following incision and drainage and subsequent contour pressure dressing, deroofing procedure or insertion of a small drainage tube into the pseudocyst with a guide needle. 2 However, pressures dressing based on traditional methods are large and incumbent for the patient. We present a clinical trial of auricular seroma managed with an innovative double helical pressure appliance which is effective and does not pose embarrassment for patient to apply.…”
