Endobronchial and Endoscopic Ultrasound-Guided Transvascular Biopsy of Mediastinal, Hilar, and Lung Lesions

Abstract: In this series, EBUS- and EUS-guided transvascular approach for biopsy of mediastinal, hilar, and lung lesions was not associated with significant complications. However, careful selection of potential candidates and close periprocedural observation are mandatory.

“…In 2012, another series of EUS guided para‐vascular station 6 lymph node sampling was described, with 12 patients undergoing successful sampling with no complications . Kazakov et al recently described their experience with trans‐aortic sampling (19/33 patients, no complications), which also appear to be solely EUS‐guided . All these reports are EUS‐guided trans‐aortic or para‐vascular station 6 punctures, with EBUS guided trans‐aortic puncture described only in reports by Pattabhiraman et al (1 case) and Mehta et al (2 cases) with no complications .…”

“…Boujaoude et al described two cases of right hilar tumor biopsied with EBUS-TBNA through the pulmonary artery: the diagnosis was reached in both cases, and no complications were reported [15]. Kazakow et al described 15 consecutive patients who underwent EBUSand EUS-guided trans-vascular biopsy in an outpatient setting [16]. Eight biopsies were done through the pulmonary artery and in seven through the aorta.…”

“…On average, the addition of transoesophageal endoscopic ultrasound (EUS) to EBUS increased sensitivity by 0.12 (95% confidence interval EBUS can also obtain tissue samples from the left adrenal glands with a transgastric approach 16 , as well as with a transvascular approach, through the pulmonary artery and aorta 17 , with no complications.…”
