Endoanchors under 3D image fusion for a type IA endoleak after EVAR

Abstract: Key Clinical Message The Heli‐FX technique for type IA EL under 3D‐IF proved to be accurate in terms of EL channel vision and correct endoanchors deployment. The EL volume rendering constant view allowed a precise anchors fixation at the EL channel. 3D‐IF confirmed to be a valid help in orientation and navigation during endovascular aortic procedure.

“…As described, this technique uses the previously implanted devices as landmarks for the FI overlay. In the literature, only one case of type IA endoleak treatment with endo-anchors deployment under redo fusion guidance is reported [31] The present study established how, in real-time fusion imaging, the distance between the target vessel in 3D volume rendering and 2D fluoroscopy is effectively reduced by using endovascular markers to overlay the images. This is due to the proximity of the previously implanted marker (stent-graft, plugs, etc.)…”

“…The procedure was carried out under three-dimensional fusion imaging in our hybrid room ( Fig 4 ), with lower costs in terms of X ray and contrast medium exposure. 14 The postoperative computed tomography (CT) scan showed the complete exclusion of the pseudoaneurysm dilatation ( Fig 5 ).

“…The second case that we report is a young man who underwent TEVAR for grade III BTAI after A proximal relining TEVAR in PLZ 3 (13) with Conformable Gore TAG Thoracic Endoprosthesis (WL Gore and Associates, AZ, USA), using a bilateral femoral percutaneous approach, was performed. The procedure was carried out under 3D Fusion imaging in our hybrid room (Figure 4), with lower costs in terms of x-ray and contrast medium exposure (14). The post-operative CT scan showed the complete exclusion of the pseudoaneurysm dilatation ( Figure 5).…”
