Background: Escherichia coli is a typical occupant of the enteric system of vertebrates. Some E. coli strains are related to urinary tract infections (UTIs) in human. E. coli strains are divided namely to the four phylogenetic groups, A, B1, B2, and D. Some investigations have indicated the relationship between phylogenetic characteristics and pathogenicity of E. coli. Thus, determining the phylogeny of unknown E. coli strains may be useful in predicting the pathogenesis. Objectives: In the present study, we aimed to compare the distribution of E. coli phylogroups in human UTIs and wild bird feces as a possible source of infection for human in a cross-sectional survey. Methods: A total of 264 E. coli isolates were obtained from human UTIs and feces of wild birds around and phylogenetic determination was carried out using the Clermont Triplex-PCR technique. Results: Our results showed that phylogenetic group B2 strains were the most prevalent in UTI cases (47.2%) followed by group D (30.2%). Group B1 contained 32.5% of the isolates in feces of wild birds, followed by group A (27.5%). There was a significant difference in E. coli phylogeny between hosts so that groups B2 and D were more prevalent in human UTIs and groups B1 and A in wild birds. Also, when comparing the phylogroups within a host, group B1 showed a higher rate in wild birds than in human UTIs. Conclusions: Although the majority of isolates from wild birds belonged to nonpathogenic phylogenetic groups B1 and A, further research seems to necessary to assess the exact relation of wild birds as pathogen sources for human by genotyping E. coli strains via high throughput genotyping assays.

Genetic Diversity and Virulence Profiles of Escherichia coli Isolates Causing Spontaneous Bacterial Peritonitis and Bacteremia in Patients with Cirrhosis

Among patients with cirrhosis, infections caused by Escherichia coli organisms that translocate from the gut are a frequent and severe complication. One hundred ten E. coli isolates from 110 cirrhotic patients with spontaneous bacterial peritonitis and/or spontaneous bacteremia were characterized for their phylogenetic group and virulence genotype (34 extraintestinal virulence factor genes). Genetic relatedness was investigated by enterobacterial repetitive intergenic consensus sequence type 2 (ERIC-2) PCR typing and multilocus sequence typing. Phylogenetic groups A, B1, B2, and D accounted for 24%, 4%, 48%, and 24% of the population, respectively. Overall, 68 distinct ERIC-2 profiles were encountered. Eleven clonal groups, represented by multiple isolates (2 to 11) from the same sequence type (ST) or sequence type complex, were identified. These clonal groups accounted for 54 (49%) isolates overall. Membership in one of these clonal groups was more frequent among B2 isolates than non-B2 isolates (67% versus 32%, P < 0.001). The most frequent sequence types were ST95 (n ‫؍‬ 13) and ST73 (n ‫؍‬ 8), followed by the ST14 and ST10 complexes (n ‫؍‬ 7). ST131 and ST69 were represented by three isolates each. Clonal group-associated isolates exhibited a greater prevalence of 11 virulence genes, including pap elements, than the other isolates. However, no association between clonal groups and host factors, type of infection, or mortality was observed. In conclusion, E. coli isolates causing spontaneous bacterial peritonitis and bacteremia in cirrhotic patients are genetically diverse. However, approximately half of the isolates belong to familiar clonal groups and exhibit extensive virulence profiles that may be associated with greater invasive potential.Infections caused by intestinal bacteria are common complications in patients with cirrhosis (8, 30). Spontaneous bacterial peritonitis (SBP) and bacteremia are the most frequent and severe infections in such patients, with in-hospital mortality rates being 20 to 30% and the survival expectancy at 6 months being Ͻ50% (1, 29, 34). Increasing evidence indicates that these infections result from bacterial translocation (BT) from the intestinal lumen to the mesenteric lymph nodes, combined with failure of host defense mechanisms to clear the translocating organisms (8, 37). BT is also associated with activation of the immune system and hyperdynamic circulatory status in cirrhosis, thereby contributing to the development of ascites and hepatorenal syndrome (37). The presence of bacterial DNA in the blood or ascitic fluid of uninfected patients, considered a surrogate marker of BT, has been shown to predict decreased 1-year survival (38). Antimicrobial Resistance in Uropathogen Isolates from Patients with Urinary Tract Infections

Abstract-Aims:Because of uncontrolled and widespread use of antibiotics, the resistance pattern of uropathogens is changing drastically, specifically in developing countries, such as Bangladesh. The aim of the study was to identify the common Urinary Tract Infection (UTI) causing pathogens in the city of Jessore, Bangladesh and to check the performance of available antibiotics used by those patients. Study Design: Random 100 UTI patients who exhibited general UTI symptoms were included in our cross-sectional study. A medical proforma was prepared to input the information associated with the experiment including symptoms of patient's age, sex, laboratory diagnosis and antimicrobial susceptibility. Place and Duration of Study: Department of Microbiology, University of Science and Technology, Jessore 7408 and Pharmacy Discipline, Life Science School, Khulna University, Khulna 9208, Bangladesh, between June 2013 and July 2014.Methodology:Urine samples from 100 suspicious urinary tract infected patients were collected as described by Thomson and Miller. Bacterial isolates were tested to identify the bacterial species and to evaluate their antimicrobial susceptibility by Kirby-Bauer disk diffusion technique against some common antibiotics. Epidata® computer program 3.1 and SPSS version 16 statistical software used for confidence interval (CI) and P value, which were defined as P value is <0.05 and CI was set at 95% level of significance for all the proportions.Results: Among 100 samples, 74 samples showed positive for cultures adversely responsible for UTIs. About five different species of uropathogenswere identified from 74 cases. Comparative prevalence ofE. coli was detected in 69 of the 100 samples (69%), while Staphylococcus spp was found in 18 samples (18%), Pseudomonas aeruginosa in 8 samples (8%), and Klebsiella pneumoniae in 6 samples (6%), respectively. Comparative antibiotic resistance profile showed that most of the strains were highly resistant to Amoxicillin (85.14%) and Cotrimoxazole (81.08%). On the contrary, the strains showed significant sensitivity to Amikacin (94.59%), Azithromycin (93.24%), Doxycycline (90.54%), and Ceftriaxone (89.18%), respectively showed significant sensitivity. Conclusion: Our results could be helpful to compel rational antibiotic use for UTI. High resistance of uropathogens to antibiotics, such as Amoxicillin and Cotrimoxaxole, has been observed in a significant number of patients in the developing world, such as Bangladesh.
