Endarterectomy, Best Medical Treatment or Both for Stroke Prevention in Patients with Asymptomatic Carotid Artery Stenosis

Abstract: Carotid endarterectomy (CEA) is currently frequently performed in subjects with asymptomatic carotid artery stenosis over 70%, as clinical trials like the Asymptomatic Carotid Atherosclerosis Study and Asymptomatic Carotid Surgery Trial demonstrated a significant benefit for stroke prevention. A low risk reduction in the long-term prevention of stroke or death and the required lower than 3% of surgical risk are associated with surgery. That means that an important number of patients needs to be operated to pre… Show more

“…These trials have, however, excluded or included only small numbers of very elderly patients, providing limited evidence about the effectiveness of the operation for these patients [9]. This represents an important gap because stroke is the second leading cause of death in individuals aged 85 and over [10] who, by 2050, will number approximately 20.9 million in the USA [11].…”

“…Many studies have investigated the association between atherosclerosis and ischemic events in large populations [15,16,17,18]. However, only a few studies have focused on the mechanical characteristics of the carotid artery, including distension, and atherosclerosis [19,20].…”
