Introduction: Breast milk is the best food to meet the nutritional needs of the child, exclusively for the first 6 months and up to two years of age, favoring the establishment of the mother-child bond. Objective: To carry out a cognitive intervention to the women who attend the “breastfeeding support group”. Method: Quantitative research, with a quasi-experimental design, in a census sample made up of 11 pregnant women and 19 breastfeeding women. The Iowa Infant Feeding Attitudes Scale (IIFAS) was used. Results: 33.30% (10) are between 33-35 years old, 63% (19) in the puerperium, 56% (17) with primary education, 60% (18) carry out agricultural / cattle raising activities, 53% (16) in free union. In the pre-intervention, the women had a positive attitude with a score of 17-48 towards artificial breastfeeding 83% (25), in the post-intervention the positive attitude towards breastfeeding changed 87% (26), with a significant difference of p <0.01, in the items of the IIFAS instrument in the pre-intervention a mean and standard deviation of 2.07 ± 1.16, with an increase in the post-intervention of 4.48±0.65, a significance difference <0.05 in the total of questions in the pre-intervention from 35.23, increased after the intervention to 76.13, their attitude changed to exclusive breastfeeding. Conclusions: the cognitive intervention increased the positive attitude towards exclusive breastfeeding, highlighting that a timely educational intervention of promotion, support and accompaniment during pregnancy and postpartum guarantees the duration and exclusivity of breastfeeding. Introducción: La lactancia materna (LM) es considerada el mejor alimento para el neonato, sin embargo, esta práctica llega a ser poco común. Es posible pensar que la actitud mostrada hacia la LM no favorece a su desarrollo, pues, quizá sea modificada por diversos factores. Objetivo: Identificar la actitud y factores asociados a la lactancia materna en madres en el área de Gineco-Obstetricia del Hospital de Alta Especialidad de Veracruz. Metodología: estudio cuantitativo, descriptivo, prospectivo, transversal, en los meses de abril a mayo 2020. La muestra del estudio fue de 134 pacientes. Para la recolección de datos se utilizó la entrevista personal, el cuestionario Iowa Infant Feeding Attitude Scale (IIFAS). Para el análisis de los factores asociados se elaboró una cédula con datos sociodemográficos. Se usó el programa Statistical Package for the Social Sciences versión 25.0 y Excel 2016, para el análisis estadístico de datos. Resultados: 34.3% de las pacientes encuestadas tienen entre 26 y 30 años, el 2.2% menos de 15. Destaca una actitud neutral hacia la lactancia materna (97%), la regresión lineal mostró que los factores asociados fueron: la escolaridad p= 0.024, tipo de parto p = 0.040 y número de controles prenatales p= 0.005. Conclusiones: La actitud de la lactancia materna está asociada a los factores escolaridad, tipo de parto y número de controles, presentando una actitud neutral hacia la lactancia materna.

Introduction: The World Health Organization (WHO) reports that in the Americas only 38% of infants are exclusively breastfed (EBF) up to 6 months. Several factors may influence the abandonment of EBF. Variables such as self-efficacy are scarcely described in EBF. In the scientific literature there are descriptive and cross-sectional studies of self-efficacy and EBF, but there are no longitudinal studies. Objective: To associate self-efficacy and EBF abandonment in women in a medical unit in Mexico. Methods: A prospective cohort study with follow-up at 5 days and 6 months postpartum. Multiple binary logistic regression (MLR) was used with variables including low self-efficacy, age >25 years old, and occupation employed. Odds ratio (OR) and 95% CI were calculated. Results: Of 212 women, 53.8% were in union and 62.7% were housewives. A low final self-efficacy was obtained with a relative risk (RR) of 2.6 [95% CI (1.05 - 6.70)] for non-exclusive EBF. MLR at the end of follow-up showed low self-efficacy with an OR of 1. 55 [95% CI (1.50 - 4.71)]; age >25 years old had an OR of 3.44 [95% CI (1.81 - 16.06)], and occupation employed had an OR of 1.13 [(95% CI (1.6 - 3.46)]. Conclusion: Low self-efficacy is a risk factor for the abandonment of EBF, so it is relevant to include it in the dissemination and promotion of EBF by the health team at the primary health care level.
