Internal medicine departments must adapt their structures and methods of operation to accommodate changing healthcare systems. The present paper discusses some challenges departments of medicine face as healthcare providers and consumers continue to change. A co-pilot model is described in this article for augmenting physicians rather than replacing them. The paper presents the co-pilot models to improve diagnoses, treatments, and monitoring. Personalized variability patterns based on the constrained-disorder principle (CDP) are described to assess chronic therapies’ effectiveness in improving patient outcomes. Based on CDP-based enhanced digital twins, this paper presents personalized treatments and follow-ups that improve diagnosis accuracy and therapy outcomes. While maintaining their professional values, departments of internal medicine must respond proactively to the needs of patients and healthcare systems. To meet the needs of patients and healthcare systems, they must strive for medical professionalism and adapt to the dynamic environment.

Contributions to the family firm literature have given attention to different challenges that family firms have to face in their life cycle when organisational complexity increases and managerial processes are required. This paper investigates how management accounting (MA) tools enable family and managerial views of doing business to complement each other during the professionalisation process. In particular, the paper discusses a longitudinal case study on a family firm’s professionalisation, when an external advisor introduced financial ratio analysis and contribution margin reports. The case evidence shows that MA tools, by facilitating communication amongst family firm members and the external advisor, favours the complementing between family firm and managerial views of doing business during the professionalisation process. When simultaneously trusting family firm and managerial views of doing business, family firm members had the chance to interpret business facts differently, discovering new business opportunities. The paper contributes to the family business literature examining the professionalisation process through the pragmatic constructivist (PC) perspective that analyses the main traits of family firm and managerial views of doing business, stressing the relevance of values in interpreting business facts and in identifying factual possibilities. On this issue, the PC perspective is useful in understanding the role of MA tools as a communication basis for enabling the complementing of different views of doing business during the professionalisation process.

Anchor practices that guide horizontal performance measurement: an interventionist case study of the financial aspect of new technology implementation in healthcare

With the demand for elderly care increasing in many countries, digital technologies offer the potential for organising such care while also increasing value for money. However, public administrators need tools to make sense of their own complex environment and the possible impacts of new technologies. The current paper examines this issue by applying horizontal performance measurement, where practitioners can give financial value to issues that span across many functions and thus avoid sub-optimisation. We use an interventionist case study to illustrate a situation in which a Nordic city attempted to calculate the financial impact of introducing new digital technologies into elderly care. As our contribution to the literature on horizontal performance measurement, we show how economic (financial) and wellbeing anchors influence horizontal performance measurement in a healthcare digitalisation project. We also contribute to the development of our method theory, i.e., anchor practices, by providing evidence of the usage of multiple simultaneous anchors and make a methodological contribution by showing that interventionist researchers can support operationalising anchor practices.
