Emergency ultrasound-guided percutaneous embolization of post-traumatic bleeding hepatic artery pseudoaneurysms

Abstract: Hepatic artery pseudoaneurysm (HAP) is an uncommon but serious complication of blunt trauma abdomen which can be managed by non-surgical interventional methods. We describe the case of a young boy with blunt trauma abdomen having a large hepatic laceration associated with two pseudoaneurysms of the hepatic artery branches. Both these HAPs were successfully managed by percutaneous injection of cyanoacrylate glue under sonographic guidance. A major surgery was thus averted. A brief review of the etiology, diagno… Show more

“…18 In our patients, we used coils and NBCA glue rather than thrombin because thrombin cannot be monitored in real time under fluoroscopy and poses a high risk of nontarget embolization or thrombosis. Some other embolic agents such as ethanol 14 or thrombin 6,12,13,[15][16][17][18]24 can be used as well, depending on the operator's familiarity with and preference for the supplies.…”

“…3 Percutaneous embolization is the second-choice procedure that allows successful embolization in cases with failure of endovascular and endoscopic methods. 4 Surgery is often reserved for unstable patients or those in whom the minimally invasive interventions fail. We present a case of postcholecystectomy right hepatic artery pseudoaneurysm at an uncommon location, near choledochoduodenal fistula, that was managed by percutaneous embolization after failure of endovascular and endoscopic procedures.…”

“…On the basis of a few case reports Based on few case reports describing the use of percutaneous glue (N-butyl-2-cyanoacrylate) embolization in abdominal wall pseudoaneurysm [1][2][3][4][5] We decided to explore the possibility of ultrasound-guided percutaneous glue embolization as a safe, effective and quick method for the treatment of acute vascular injuries in cirrhotic patients with active haemorrhage as compared with standard arterial embolization.…”
