Emergency Transcranial Doppler Assessment of Intracranial Vertebral Artery Dissection

Abstract: Despite its risks, contrast angiography is the method of definitive diagnosis of vertebral artery dissection. This presents the case of a 49‐year‐old white man with ischemic symptomatology in the posterior circulation in whom transcranial Doppler sonography suggested vertebral dissection, leading to confirmation by emergency angiography and rapid institution of therapy.

“…Recently developed noninvasive techniques allow complications as described in patient number 13 to be avoided. The first step should be (depending, of course, on local availability) a neurovascular examination using TCD and duplex ultrasonography 21 . We have described the diagnostic potential of the color‐flow duplex ultrasonography in the evaluation of extracranial vertebral artery dissection in another study 15,22 .…”

“…In some previously published reports, Doppler ultrasonography was found to be a sensitive method for the detection of pathologic conditions of the vertebral artery. 15,26 However, most of our TCCS findings were also nonspecific. Definitively abnormal findings included absence of flow, reduced velocity, and reversed direction of flow.…”
