Em2-ELISA for the follow-up of alveolar echinococcosis after complete surgical resection of liver lesions

Abstract: Alveolar echinococcosis, a serious and often fatal human disease, can be efficiently cured only by complete surgical resection of the Echinococcus multilocularis lesion. The present study showed that the determination in patients who had undergone surgery of antibody activity directed against the antigen Em2 reliably reflected complete or incomplete surgical resection. From 9 patients with pre-operative positive results in the Em2 enzyme-linked immunosorbent assay (Em2-ELISA) and successful surgical resection,… Show more

“…E. granulosus and E. multilocularis share many common antigens, but for the differential diagnosis the specific E. multilocularis antigens Em2+ and Em18 are recommended for diagnosis of human AE (Gottstein et al, 1989;Ito et al 1995). In our study, the concentration of total IgG antibodies (expressed as antibody units in %) to Em2+ antigen was in a high correlation with IgG to EmP antigen during the follow up of all patients.…”

“…The standard control values for IgG and IgG4 to EmP antigen were the highest ratios between the positive and negative controls. Finally, the recorded OD values were used to calculate antibody units (AU) as the percentage of the positive standard control values accord-ing to Gottstein et al (1989). Levels of specific IgG and IgG4 antibodies obtained with EmP antigen are presented in Table 1.…”

“…22 Conventional CHD serology is based primarily on a screening test such as an EgHF-ELISA and a subsequent confirmation test such as arc-5-electrosyneresis or immunoblotting for the 8-kD antigen. 9 In contrast to AE or cysticercosis serology, [23][24][25] relatively few immunodiagnostic studies have been reported that reliably address routine application and evaluation of the diagnostic test. In central Europe, these include primarily the differential diagnosis of AE, cysticercosis, and other parasitic infections with potential cross-reactivity, other hepatic disorders including malignancies that may mimic cyst-like structures in imaging procedures, and malignancies that may putatively lead to false-positive reactions such as those described by Dar and others.…”
