Eagle’s syndrome is a collection of symptoms due to abnormal and/or elongated styloid process. This can irritate the various neurovascular structures that lie in its close proximity, mainly the glossopharyngeal nerve, leading to odynophagia, dysphagia, foreign body sensation, cervicofacial pain, and headache. It is a diagnosis of exclusion and needs high degree of clinical suspicion. It is a rare condition with no reported incidence in the Bhutanese population so far. In this article, we present a case of elongated styloid process that was causing persistent and troublesome orofacial pain in a patient, which was not relieved by medication. The patient underwent trans-oral styloidectomy, which helped cure his symptoms.

В статье приводятся топографические, анатомические и морфометрические особенности шиловидного отростка, шилоподъязычной связки, малых рогов подъязычной кости, которые функционально объединяются в шилоподъязычный комплекс. Топография шиловидного отростка височной кости (ШОВК) имеет ряд особенностей. Его дистальная часть проходит в окологлоточном пространстве шеи вдоль стенки глотки, где сосудисто-нервный пучок шеи анатомически тесно прилежит к шиловидному отростку. Заостренный конец удлиненного и искривленного ШОВК может раздражать окружающие его нервные, сосудистые и мышечные структуры. Набор клинических симптомов, связанных с удлинением или искривлением ШОВК, полным или частичным окостенением шилоподъязычной связки с образованием суставоподобных сочленений определяется как шилоподъязычный синдром (ШПС) или синдром Eagle. Это общепринятое название для набора клинических симптомов, связанных с аномалиями размеров и положения ШОВК. Основой патогенеза ШПС является не только удлинение отростка, но и его патологическое отклонение в медиально-заднем направлении в сочетании с утолщением и искривлением. В зависимости от того, какое из расположенных в непосредственной близости образований подвергается воздействию верхушки отклоненного отростка, выделяют два подвида ШПС: шиловидно-глоточный и шиловидно-каротидный. Мы уделили больше внимания шилоглоточному синдрому. Несмотря на высокие исследовательские технологии ШПС до сих пор мало известен практическим врачам и недостаточно изучен, хотя аномалии шилоподъязычного комплекса встречаются у 20—30% людей. The article presents the topographic, anatomical, and morphometric features of the styloid process, stylohyoid ligament, lesser horns of the hyoid bone, which are functionally combined into the stylohyoid complex. The topography of the styloid process of the temporal bone has several features. The distal part of it passes in the peripharyngeal space of the neck along the pharyngeal wall, where the neurovascular bundle of the neck is anatomically closely adjacent to the styloid process. The pointed end of the elongated and curved styloid process of the temporal bone can irritate the surrounding nerve, vascular and muscle structures. A set of clinical symptoms associated with lengthening or curvature of the styloid process of the temporal bone, complete or partial ossification of the stylohyoid ligament with the formation of articular joints is defined as stylohyoid syndrome or Eagle syndrome. This is the generally accepted name for a set of clinical symptoms associated with abnormalities in the size and position of the styloid process of the temporal bone. The basis of the pathogenesis of the stylohyoid syndrome is not only the elongation of the process, but also its pathological deviation in the medial-posterior direction, combined with thickening and curvature. Depending on which of the formations located in the immediate vicinity is exposed to the apex of the deviated process, two subspecies of the stylohyoid syndrome are distinguished: styloid-pharyngeal and styloid-carotid. We paid more attention to the stylo-pharyngeal syndrome. . Despite high research technologies, stylohyoid syndrome remains little known to practitioners and an insufficiently studied disease, although anomalies of the stylohyoid complex occur in 20—30% of people.
