Drug hypersensitivity reactions (DHRs) are nowadays the third cause of allergy after rhinitis and asthma with a significant increase in prevalence in both adults and paediatric population with new drugs included as culprit. For this, DHRs represent not only a health problem but also a significant financial burden for affected individuals and health systems. Mislabelling DHRs is showing to be a relevant problem for both, false label of drug allergic and false label of nonallergic. All this reinforces the need to improve accurate diagnostic approaches that allow an appropriate management.Moreover, there is a need for training both, nonallergist stakeholders and patients to improve the reaction identification and therefore decrease the mislabelling. The use of allergy cards has shown to be relevant to avoid the induction of DHRs due to the prescription of wrong medication.Recent developments over the last 2 years and highlights about risk factors, diagnostic approaches, mechanisms involved as well as prevention actions, and management have been reviewed. In these papers, it has been outlined the need for correct diagnosis and de-labelling of patients previously false-reported as allergic, which will improve the management and treatment of patients with DHRs. Decision support consists in helping a decision-maker to improve his/her decisions. However, clients requesting decision support are often themselves experts and are often taken by third parties and/or the general public to be responsible for the decisions they make. This predicament raises complex challenges for decision analysts, who have to avoid infringing upon the expertise and responsibility of the decision-maker. The case of diagnosis decision support in healthcare contexts is particularly illustrative. To support clinicians in their work and minimize the risk of medical error, various decision support systems have been developed, as part of information systems that are now ubiquitous in healthcare contexts. To develop, in collaboration with the hospitals of Lyon, a diagnostic decision support system for day-today customary consultations, we propose in this paper a critical analysis of current approaches to diagnostic decision support, which mainly consist in providing them with guidelines or even full-fledged diagnosis recommendations. We highlight that the use of such decision support systems by physicians raises responsibility issues, but also that it is at odds with the needs and constraints of customary consultations. We argue that the historical choice to favor guidelines or recommendations to physicians implies a very specific vision of what it means to support physicians, and we argue that the flaws of this vision partially ex-A.

Evaluation Criteria for the Effects of Decision Support Integrated into Computerized Provider Order Entry System: A Scoping Review

Introduction: This study was done to extract the evaluation criteria to assess the effects of decision support systems integrated with computerized provider order entry (CPOE) systems. Methods: A Scoping review search was carried out on papers published in nine electronic databases, including PubMed, Embase, ProQuest, Scopus, Web of Science, Cochrane, Science Direct, ACM digital library, and IEEE Xplore Digital Library up to February 2019. This study was conducted based on the PRISMA flow diagram. Two investigators independently worked on identifying papers published in English electronic clinical decision supports physicians used that to help decision-making during medical orders. Finally, the criteria for effects of clinical decision support systems (CDSSs) in CPOE were extracted from the selected papers. Results: Eighty-seven studies were identified matching the inclusion criteria. The most significant number of effects belonged to the medication order decision support system. Medication order decision support system studies were classified into five categories by effects: clinical effects (8 dimensions), the process of care effects (3 dimensions), user workload effects (8 dimensions), economic effects (2 dimensions), and implementation effects (5 dimensions). Conclusions: It can be concluded that the most substantial effect is related to medication decision supports within the CPOE system. These studies provide wide-ranging criteria to evaluate CDSS integrated into CPOE. It helps identify weaknesses and strengths of CDSSs within CPOE systems.
