Паттерн «вспышка-подавление» (ПВП, burst suppression), классифицируемый как участки подавленной активности, которые составляют более 50% всей записи и постоянно перемежаются со вспышками нормальной амплитуды, относится к наиболее выраженным нарушениям на электроэнцефалограмме. Регистрацию ПВП принято в мировой клинической практике относить к так называемым злокачественным паттернам, выявление которых связано с абсолютно неблагоприятным прогнозом восстановления сознания, за исключением случаев использования веществ, угнетающих центральную нервную систему. Эти данные относятся к взрослой популяции, в педиатрической практике феномен исследован мало. Мы приводим случай восстановления сознания и нормализации клинического состояния у девочки 5 лет с вирусным энцефалитом, у которой в течение 7 сут регистрировался ПВП. Таким образом, регистрация ПВП у детей в критическом состоянии отражает глубокое нарушение биоэлектрической активности головного мозга, но не является абсолютно неблагоприятным прогнозом, и при успешном проведении надлежащей интенсивной терапии состояние пациента может улучшаться вплоть до полного восстановления. Приведенный пример подтверждает необходимость, во-первых, осуществления длительного и частого электроэнцефалографического исследования у детей в критических состояниях для объективной оценки динамики изменения биоэлектрической активности, во-вторых-исследования у таких пациентов соматосенсорных вызванных потенциалов (ССВП) при стимуляции срединных нервов. Высокая точность и информативность ССВП для прогнозирования неблагоприятного исхода заболевания, сопровождающегося комой, подтверждены неоднократно, а чувствительность и специфичность метода не зависят от использования препаратов, угнетающих нервную систему.
