Electrocautery Resection, Shaving with a Scalpel, and Podophyllin: a Combination Therapy for Giant Condyloma Acuminatum

Abstract: Giant condyloma acuminatum (also known as Buschke-Löwenstein tumor) is a rare disease, for which the treatment of choice is still controversial. In the case described in this report, we used a combination of electrocautery and a surgical scalpel to resect a huge genital wart, followed by long-term local treatment with podophyllin. This procedure was relatively fast, easy to perform, involved minimal bleeding, and did not result in any complications. Moreover, combination therapy enabled us to avoid deep resect… Show more

“…3 Wide surgical excision has been recommended as the treatment of choice. 1,3 However, in cases where considerable tissue cannot be excised to retain sexual function, non-surgical modalities should be tried. 4 The latter comprises intralesional immunotherapy, topical agents like 5% imiquimod, podophyllin resin 25%–30%, trichloroacetic acid, physical ablative modalities like cryotherapy, carbon dioxide laser, electrocautery and systemic therapies including interferon-α, bleomycin, methotrexate and 5-fluorouracil.…”

“…Giant condyloma acuminatum (GCA) is a unique and uncommon variant of anogenital warts, accounting for 0.1% of the cases. 1 It exhibits a locally aggressive behaviour, albeit with benign histopathological features. Treatment of GCA is often unsatisfactory and local recurrence is common.…”

“…1 Deve-se enfatizar que a reconstrução de retalho local pode melhorar significativamente a qualidade de vida após a excisão cirúrgica ampla, e um acompanhamento regular é necessário devido às recorrências frequentes e possíveis metástases a distância.…”

“…Most of the incidence attacks middle-aged men, with a male-to-female ratio at 3:1. 2 Risk factors for GCA include anoreceptive intercourse, Human Immunodeficiency Virus (HIV), and immunosuppression. 3 Human Immunodefi ciency Virus infection is a predisposition that increases the CA incidence and spread caused by HPV.…”
