Giant condyloma acuminatum occurs infrequently in children and adolescents. It is a challenging condition which is difficult to treat with both surgical and medical modalities. We present a case of a 15-year-old boy with giant condyloma acuminatum of the glans penis refractory to several therapeutic modalities. The lesions were treated successfully with a novel combination of Mycobacterium indicus pranii immunotherapy and acitretin. There was no recurrence of lesions after two years of follow-up.

RESUMOO condiloma gigante de Buschke-Lowenstein é uma doença sexualmente transmissível causada pelo papilomavírus humano, sendo caracterizado clinicamente por verrugas genitais, com alto potencial de se transformar em carcinoma espinocelular. Este artigo busca relatar o caso de um paciente do sexo masculino, HIV negativo, que apresentava sangramento, saída de secreção purulenta e lesão com aspecto vegetante em região perianal. A biópsia da lesão evidenciou condiloma de Buschke-Lowenstein e, após seis meses, foi realizada a cirurgia com ampla excisão da lesão e amputação retal. A análise anatomopatológica da peça cirúrgica mostrou carcinoma espinocelular. Palavras-chaves: tumor de Buschke-Lowenstein; condiloma acuminado; carcinoma verrucoso; neoplasias de células escamosas; papiloma. ABSTRACTThe giant condylomata acuminata of Buschke-Lowenstein is a sexually transmitted disease caused by the human papillomavirus, clinically characterized by genital warts, with high potential to develop into squamous cell carcinoma. This article seeks to report the case of a male patient, HIV-negative, who had bleeding, purulent discharge and injury with vegetative aspect in the perianal region. Biopsy of the lesion showed condyloma of Buschke-Lowenstein, and, after six months, surgery with wide excision of the lesion and rectum amputation was performed. The anatomopathological examination of the surgical specimen showed squamous cells carcinoma. INTRODUÇÃOO condiloma acuminado é uma doença sexualmente transmissível causada pelo papilomavírus humano (HPV), principalmente HPVs 6 e 11, sendo caracterizado clinicamente por verrugas genitais. Raramente, as verrugas genitais podem evoluir para grandes massas anogenitais, definindo a forma gigante dessa doença, 1 a qual é conhecida como tumor de Buschke-Lowenstein, que recebe esse nome porque foi descrito por Buschke e Lowenstein em 1925, como uma lesão de pênis.2 É uma forma rara de verrugas genitais, 3 compreendendo não mais do que 0,1% dessas, 1 caracterizando-se como um grande tumor devido ao excessivo crescimento epitelial, com aspecto de couve-flor. Na literatura, os dados sobre a prevalência da infecção por HPV em homens variam significativamente entre 1,3 e 72,9%; além disso, a distribuição exata dos genótipos específicos do HPV nessa população ainda não está clara, 4 sendo a doença sexualmente transmissível mais comum da região anorretal. 5O período de incubação do HPV é em torno de um a seis meses. Ainda que a lesão seja histopatologicamente benigna, exibe propriedades clinicamente malignas devido ao crescimento local, que cria pressão sobre o tecido circundante, falta de regressão espontânea e uma alta taxa de recorrência, apesar do tratamento. Perianal giant condylomata acuminate (GCA) is a rare clinical condition associated with low-risk Human papillomavirus (HPV) type 6 and 11 infections. Human Immunodeficiency Virus (HIV) infection is one of the risk factors for GCA, that can increase the condylomata acuminate incidence and spread caused by HPV. A 28-year-old man came with a cauliflower-like mass complaint in his perianal and anal since 2 months ago. The patient did not complain of pain or itching on the mass but often bled when defecating. The patient is a male who has sex with men (MSM) and often changes partners. He has been diagnosed with HIV since 11 months ago and regularly taking anti-retroviral drugs, Efavirenz 600 mg daily. He was also diagnosed having lung tuberculosis at the same time, got 6 months treatment and was declared cured. The venereological examination of the perianal and anal region revealed erythematous and grayish stem-shaped vegetation and papules, verrucous surface, multiple, well defined, with 3 x 1.5 x 2 cm in size. A positive act of white examination was obtained. Blood tests revealed CD+4 230 cells /μL. Polymerase chain reaction (PCR) examination for HPV obtained HPV types 6 and 11 infections. Histopathologic examination revealed acanthosis, papillomatosis, and hyperkeratotic epidermis and koilocytotic cells. The patient was treated with electrodesiccation three times but obtained mass in anal getting bigger with a size of 6 x 3 x 3 cm. Therefore, he agreed to be referred to the surgical department with an extensive surgical excision plan. Screening of GCA using PCR is not a routine examination but PCR has high sensitivity and specificity for determining the type of HPV, is useful for determining GCA prognosis and therapy, and is recommended for malignant and possible GCA recurrence detection
