Electrocardiographic-Gated Multislice Computed Tomography for Visualization of Cardiac Morphology in Congenitally Corrected Transposition of the Great Arteries

Abstract: A case of congenitally corrected transposition of the great arteries in a 64-old-woman is presented. Diagnosis was missed by invasive angiocardiography. Electrocardiographic-gated multislice computed tomography not only demonstrated switching of the aortic root and pulmonary trunk but clearly identified fine morphologic details of the cardiac chambers, including the atypical coronary artery pattern.

“…Although echocardiography usually remains the primary imaging technique in congenital heart disease, comprehensive anatomic and functional analysis can not be obtained by its [11]. In addition, it is an operator-dependent method and may present difficulties of older children, adults, and patient who have undergone surgery [9,13]. MR angiography can depict the complex anatomy of the patient with CCTGA and can be used to identify [11,14].…”

“…Dextrocardia occurs in approximately 25% of patients [7]. Because the morphologic RV pumps the systemic circulation, the RV shows adaptive myocardial hypertrophy, and develop right-sided heart failure [9]. Regurgitation of the systemic AV valve (tricuspid valve) develops in about 30% of patients and in most patients may be cause of the systemic ventricular insufficiency.…”

“…Echocardiography is the primary imaging technique in congenital cardiac diseases, but in this case, comprehensive anatomic and functional analysis could not be obtained 6 . In addition, it is an operator‐dependent method and may present difficulties due to limited acoustic imaging windows in older patients and patients who have undergone surgery 7 …”

“…The present complicated classifications of CA in the literature [19][20][21][22] should be grouped into fewer and briefer patterns; it is time to unify and simplify the complex CA classification of all the normal and malformed hearts according to APR into the six patterns we reported herein and previously [9][10][11]13]. Hopefully, this classification can be cited gradually [23][24][25].…”
