Background: Hysterectomy, is a common operation worldwide. It is a major surgery, which leads to significant morbidity and mortality, hence, there must be a justifiable indication before the procedure is undertaken. Symptomatic uterine fibroid and utero-vaginal prolapse are common indications in this environment. Objectives: To determine the prevalence, indications and outcome of hysterectomy at Aminu Kano Teaching Hospital, Kano. Study Design: A retrospective study of all the hysterectomies that were done in Aminu Kano Teaching Hospital, Kano between 1st of Jan 2009 and 31st of Dec 2013. Result: During the period of study, there were 3843 major gynaecological operations. Out of which hysterectomy was 196, giving hysterectomy rate of 5.1% of all major gynaecological operations. Hysterectomy was highest among patient aged 40-49 years accounting for 54.6% of cases and the commonest indication was uterine fibroid, constituting 66.4% of all hysterectomies. Total abdominal hysterectomy was the most frequently preformed, accounting for 78.1% while, vaginal hysterectomy accounts for 20.9% of cases and subtotal hysterectomy was 1%. Hysterectomy was mostly complicated with post-operative pyrexia constituting 24% of all the complications while ureteric injury was the least, constituting 1%. There was no mortality recorded. Most of the patients who had hysterectomy were discharge in less than 10th post-operative day accounting for 91.3% of cases. However, 80.5% of patients who had vagina hysterectomy were discharge in less than 5th post-operative day. Conclusion: The prevalence of hysterectomy was 5.1%. Uterine fibroid was the commonest indication while postoperative pyrexia was the commonest complication found.

Most of the women in our environment abhor hysterectomy but it remains the only option in most cases in our practice of gynaecology due to late presentation when it becomes more difficult to offer a less radical management options. This study aims to evaluate the indications, clinical characteristics and management outcome in the Niger Delta of Nigeria. This is a retrospective analysis of all Hysterectomies performed from 1st January, 2000 through 31st December, 2005 at the University of Port Harcourt Teaching Hospital. There were 170 cases of elective hysterectomy out of a total of 1025 elective major gynaecological surgeries during the study period with hysterectomy prevalence of 16.6%. The mean age of the patients was 46.5 years, 21(13.55%) of the women were nulliparous. The major indications were symptomatic uterine fibroid 55.5%, ovarian tumour 14.8% and uterovaginal prolapsed 8.4%. The surgery in 92 (59.4%) patients was performed by consultants. The crude morbidity and case fatality rates was 45.45% and 3.87% respectively. Elective hysterectomy for gynaecological condition remains a valid management option in our environment. The use of peri-operative antibiotics is advised to reduce the incidence of post-operative morbidity.

A 5-year review of management of uterine fibroids at the university of Nigeria Teaching Hospital Enugu revealed that symptomatic uterine fibroids constituted 9.8% of all gynaecological admissions. The peak incidence n = 51 (26.8%) was in the age range of 31 - 35 years and the majority of the women n = 77 (40.5%) were nulliparous. The main clinical features were lower abdominal discomfort, menorrhagia and associated infertility. There was no medical management. The surgical management consisted of myomectomy by laparotomy (60%); total abdominal hysterectomy (24.7%); total abdominal hysterectomy and bilateral salpingo-oophorectomy (12.1%), and polypectomy (3.2%). There was no endoscopic surgery available. Postoperative morbidity was high but there was no mortality. Uterine fibroids mostly managed by myomectomy at laparotomy remains a major public health problem in Enugu Nigeria.
