Objective: Compare detrusor muscle of normal and patients with infravesical obstruction, quantifying the collagen and elastic system fibers. Materials and Methods:We studied samples taken from bladders of 10 patients whose ages ranged from 45 to 75 years (mean = 60 years), who underwent transvesical prostatectomy for treatment of BPH. Control material was composed of 10 vesical specimens, removed during autopsies performed in cadavers of accident victims, with ages between 18 and 35 years (mean = 26 years). Results:The results of collagen and elastic fibers quantification (volumetric density) demonstrated the following results in percentage (mean +/-standard deviation): collagen in BPH patients = 4.89 +/-2.64 and 2.32 +/-1.25 in controls (p < 0.0001), elastin in BPH patients = 10.63% +/-2.00 and 8.94% +/-1.19 in controls (p < 0.0001). Conclusion:We found that the components of connective tissue, collagen and elastic system fibers are increased in the detrusor muscle of patients with infravesical obstruction, when compared to controls.

The injectable and hydrophilic nature of hydrogels makes them suitable candidates for cartilage tissue engineering. To date, a wide range of hydrogels have been proposed for articular cartilage regeneration but few studies have quantitatively compared chondrocyte behaviour and extracellular matrix (ECM) synthesis within the hydrogels. Herein we have examined the nature of ECM synthesis by chondrocytes seeded into four hydrogels formed by either temperature change, self-assembly or chemical crosslinking. Bovine articular cartilage chondrocytes were cultured for 14 days in Extracel®, Pluronic F127 blended with Type II collagen, Puramatrix® and Matrixhyal®. The discriminatory and sensitive technique of fluorophoreassisted carbohydrate electrophoresis (FACE) was used to determine the fine detail of the glycosaminoglycans (GAG); hyaluronan and chondroitin sulphate. FACE analysis for chondroitin sulphate and hyaluronan profiles in Puramatrix® closely matched that of native cartilage. For each hydrogel, DNA content, viability and morphology were assessed. Total collagen and total sulphated GAG production were measured and normalised to DNA content. Significant differences were found in total collagen synthesis. By day 14, Extracel® and Puramatrix® had significantly more total collagen than Matrixhyal® (1.77±0.26 μg and 1.97±0.26 μg vs. 0.60±0.26 μg; p<0.05). sGAG synthesis occurred in all hydrogels but a significantly higher amount of sGAG was retained within Extracel® at days 7 and 14 (p<0.05). In summary, we have shown that the biochemical and biophysical characteristics of each hydrogel directly or indirectly influenced ECM formation. A detailed understanding of the ECM in the development of engineered constructs is an important step in monitoring the success of cartilage regeneration strategies.

Chronic urinary retention (CUR) is one of the consequences of bladder outlet obstruction (BOO) that occurs due to impaired functions of detrusor muscles secondary to obstruction related changes in the bladder wall. The effect of catheter drainage (CD) on detrusor function in CUR in animals is well studied but in humans there are very few studies to see its effect in CUR. Material and methods:This is a non-randomized, retrospective study wherein case records of 50 eligible patients attending the Department of Urology during January 2010 to March 2012 were evaluated for CUR. Two UDS, performed 6-12 weeks (average 9 weeks) apart and the parameters which were evaluated were bladder sensation, capacity and voiding detrusor pressure. These parameters were also evaluated against the age of the patient, duration of symptoms and the amount of urine drained on catheterization. Results:The commonest symptoms observed were decrease flows of urine, frequency, straining and incomplete emptying. Bladder function improvement after CD was observed to be maximum in patients with following characteristics: age at presentation in the range of > 60 to < 70 years, duration of lower urinary tract symptoms (LUTS) less than 5 years and amount of urine drained at the time of catheterization being < 500 ml. Conclusion:CUR secondary to BOO is a common problem in elderly males. UDS is the "gold standard" for its evaluation and helps to plan further management. CD of urine is an effective mode of temporary management in patients of CUR secondary to BOO as it provides rest to the detrusor muscle in humans.
