Background: Acute rheumatic fever (ARF) is an important cause of valvular heart disease in children. Endothelial dysfunction plays an important role in the pathogenesis of valvular heart diseases. The role of endothelial dysfunction in valvular heart diseases due to ARF is not exactly known. In ARF, autoimmune injury, inflammation, oxidative stress, and impairment of nitric oxide in valvular endothelium may be the causes of endothelial dysfunction. The purpose of this study is to evaluate endothelial dysfunction and arterial stiffness in children with ARF. Materials and Methods: Thirty-six patients diagnosed with ARF (the mean age was 11.80 ± 2.82 years) and 36 volunteered individuals with similar age, sex, and body mass index were included in the study. The study groups were compared in terms of M-mode echocardiography parameters, carotid arterial strain (CAS), beta-stiffness index (βSI), and flow-mediated dilation (FMD). Results: In patients with ARF, there was a decrease in FMD% (10.36 ± 7.26 and 12.76 ± 4.59; P < 0.001) compared to the control group. In addition, CAS (0.16 ± 0.06 and 0.18 ± 0.08; P = 0.44) and βSI (3.65 ± 1.61 and 3.57 ± 2.38; P = 0.24) were similar in the patient and the control groups. Furthermore, no correlation was detected between decreased FMD value and mitral regurgitation ( r = −0.07; P = 0.66), aortic regurgitation ( r = −0.04; P = 0.78), CAS ( r = −0.08; P = 0.61), and βSİ ( r = −0.20; P = 0.22). Conclusion: In our study, a decrease in FMD value, which is a marker of endothelial dysfunction, was found in children with rheumatic carditis.

Aortic stiffness studies in children with Kawasaki disease: preliminary results from a follow-up study from North India

Kawasaki disease (KD) is an acute medium vessel vasculitis seen in children. Coronary artery abnormalities are the most notable long-term complications. Intravenous immunoglobulin therapy has led to significant reduction in incidence of coronary aneurysms. However, higher risk of cardiovascular disease has been noted even in those children who have never been reported to develop coronary artery aneurysms during the subacute phase. Noninvasive estimation of this cardiovascular risk is useful in initiating timely preventive strategies. Arterial stiffness has been shown to be an independent predictor of cardiovascular events in general population. This study was aimed at measuring abdominal aorta stiffness in a cohort of children with KD from North India. Thirty children diagnosed to have KD at least 5 years prior to enrollment were evaluated by echocardiography for cardiac and abdominal aorta studies. Electrocardiographs were done in all patients for QT dispersion. There was a noticeable trend toward lower aortic strain, higher pressure strain elastic modulus and normalized pressure strain elastic modulus in children in the study group, though this difference did not reach statistical significance. Mean corrected QT dispersion was significantly higher in cases compared with controls. On cardiac evaluation, none of the subjects had any residual coronary artery involvement. A trend toward lower distensibility and higher stiffness of abdominal aorta was seen in cases compared with controls, though it was not found to be statistically significant. This could be a predictor of premature atherosclerosis in long term. Significantly higher QT dispersion was seen in cases compared with controls. More multicentric studies with larger sample size and longer follow-up periods are needed to clarify this issue.

Purpose: Bicuspid aortic valve (BAV) is the most common congenital cardiac anomaly in adults. Although there is some publication about the frequency of BAV, there is limited data about BAV in adult patients in our country. The aim of our study was to determine the prevalence and echocardiographic characteristics of BAV patients who underwent echocardiography. Materials and Methods: To determine patients with BAV, three and half year echocardiographic data in our adult cardiology clinic was retrospectively examined in this retrospective study. Demographic features and laboratory findings were also obtained from hospital record. Student t-test, chisquare test and multivariate linear regression analyses were used for the statistical analysis. Results: Among 52742 patients, BAV prevalence was found to be 4.4 (n=235) in 1000 patients. The median age of BAV patients was 46.4±17.6 years and 159 (67.7%) patients were male. Ascending aorta dilatation was found in 117 (51%) patients with BAV. BAV patients with aortic dilatation were significantly older (53.9±14.9 vs. 39.3±17.1, p=<0.001, respectively). Male gender were frequently seen in patients with aortic dilatation as compared without (%73.5 vs. %60.7; p= 0.039, respectively). Sixty-five (27.7%) of the BAV patients had aortic stenosis and echocardiographic graded as mild in 36 (15.3%), moderate in 20 (8.5%) and severe in 9 (3.5%) patients. Aortic regurgitation was observed in 185 (78.7%) out of the BAV patients and echocardiographic graded as mild in 144 (61.3%), moderate in 27 (11.5%) and severe in 14 (6.0%) patients. Conclusion: The prevalence of BAV diagnosed by echocardiography at the adult cardiology clinic was 0.44%. Additionally, it was found that male gender and age were the most significant parameters associated with aortic dilatation.
