Introduction: Rupture of the diaphragm is almost always due to major trauma. Diaphragmatic injuries are rare (5-7%), usually secondary to blunt, or more rarely to penetrating, thoracic or abdominal trauma. No single investigation provides a reliable diagnosis of diaphragmatic rupture when a patient first arrives at hospital. Almost 33% are suspected on initial chest x-ray, but the percentage is lower in patients who are immediately intubated. Mortality in patients with diaphragmatic rupture following blunt abdominal trauma is generally associated with coexistent vascular and visceral injuries that could be rapidly fatal. It's mandatory that the right diagnosis is reached as soon as possible given that mortality is influenced by the time elapsing between trauma and diagnosis.

El trauma cerrado de abdomen sigue siendo una de las principales fuentes de ingreso a sala de trauma en los hospitales. Se manifiesta principalmente en personas jóvenes, especialmente del sexo masculino, producoto de accidentes de tráfico. Su manejo desde el diagnóstico hasta el tratamiento es clave para garantizar la supervivencia del paciente. Se realizó una revisión de literatura, de un total de 25 artículos, publicados en español e inglés, en plataformas como PubMed, Cochrane, Springer, ELSEVIER, Virtual de Salud (BVS) y Google Académico. El diagnóstico es fundamental para reducir la mortalidad en el trauma cerrado de abdomen. La ecografía FAST y la tomografía computada multicorte son los procedimientos de primer nivel para un mejor diagnóstico. El bazo, el hígado y el mesenterio son los órganos con mayores daños. Se pueden manifestar complicaciones asociadas con trauma torácico o de la región pélvica. El tratamiento conservador, laparoscopia exploratoria o quirúrgica y la laparotomía tota son los principales tratamientos del trauma abdominal cerrado. Gracias a las mejoras en los métodos diagnóstico, se ha logrado reducir la mortalidad o el riesgo intrahospitalario por laparotomía exploratoria negativa.

Ectopic pancreas (EP) is a rare congenital anomaly defined as the presence of pancreatic tissue in topographic anomaly. It is usually silent but it may become clinically evident when complicated by acute conditions. The development of laparoscopic surgery has changed the way to manage such conditions, especially in the setting of emergency surgery, thanks to its diagnostic and therapeutic role with excellent results. We decided to perform an emergency diagnostic exploratory laparoscopy in a 29-year-old man with an acute abdomen and nonspecific radiological images for intestinal occlusion. A jejunojejunal intussusception was found, caused by a mass. We decided to carry out minilaparotomy to perform a resection of the affected jejunum. Histological examination confirmed the presence of a jejunal ectopic pancreas. Adult intussusception caused by EP represents 5% of all cases of intussusception. As CT scan, especially when performed in emergency setting for small bowel obstruction diagnosis, can usually demonstrate nondiagnostic findings suggestive of intussusception of unknown origin, laparoscopic exploration could help surgeons in order to perform a resolute diagnosis and treat the pathology.
