Einsatz der Sonographie in der Diagnostik der frischen fibulo-talaren Bandläsion

Abstract: Sonography of the ankle joint is a simple method of exploring articular soft tissue. 100 patients with acute injury of the lateral ligaments were examined with a 7.5 MHz linear array transducer. Diagnostic criteria were haematoma and capsular deformation. Sonographic findings were assessed by stress films (n = 94), arthrography (n = 29) and operative evaluation (n = 28). In 94 of 100 joints, acute injury could be demonstrated with sonography. In most cases sonography was able to differentiate the site of the l… Show more

“…Furthermore, they do not enable direct visualization of the injured ligaments, nor a reliable differentiation between the rupture of a ligament and that of the joint capsule; the latter is a common cause of false-positive arthrographic findings [3,4,12,13]. Sonography has been suggested for assessment of the ankle joint and for direct evaluation of ligament morphology; however, references are sparse and its role still remains limited [14,15].…”

“…In previous investigations of the ankle joint, the lateral collateral ligaments and the anterior tibiofibular ligament could not be visualized directly with sufficient clarity by 7.5 MHz ultrasound-probes (Friedrich et al 1990, Striepling et al 1991. This is attributable to an inadequate axial resolution of about 0.4 mm in relation to the 1 or 2 mm thickness of the ligament.…”

“…Im Gegensatz hierzu ist die Sonografie eine kostengünstige, schnelle und flächenweit verfügbare Untersuchungsmethode. In Studien wurde die Eignung der Sonografie bei der Diagnostik von akuten Verletzungen des lateralen Bandapparats des Sprunggelenks bereits bestätigt [21,23]. Jedoch ist die Sonografie eine dynamische Untersuchungsmethode, die praktische Expertise verlangt [10] Lig.…”
