Objectives To assess the effects of treatments for non-metastatic invasive squamous cell carcinoma (SCC) of the skin using evidence from observational studies, given the paucity of evidence from randomised controlled trials.Design Systematic review of observational studies. Medline, Embase, to December 2012. Review methods Observational studies of interventions for primary, non-metastatic, invasive, SCC of the skin that reported recurrence during follow-up, quality of life, initial response to treatment, adverse events, cosmetic appearance, or death from disease. Studies were excluded if data for primary cutaneous SCC was not separable from other data. Data were extracted independently by two reviewers. Meta-analysis was performed where appropriate using a random effects model to estimate the pooled proportion of an event with 95% confidence intervals. Data sourcesResults 118 publications were included, covering seven treatment modalities. Pooled estimates of recurrence of SCCs were lowest after cryotherapy (0.8% (95% confidence interval 0.1% to 2%)) and curettage and electrodesiccation (1.7% (0.5% to 3.4%)), but most treated SCCs were small, low risk lesions. After Mohs micrographic surgery, the pooled estimate of local recurrence during variable follow-up periods from 10 studies was 3.0% (2.2% to 3.9%), which was non-significantly lower than the pooled average local recurrence of 5.4% (2.5% to 9.1%) after standard surgical excision (12 studies), and 6.4% (3.0% to 11.0%) after external radiotherapy (7 studies). After an apparently successful initial response of SCCs to photodynamic therapy, pooled average recurrence of 26.4% (12.3% to 43.7%; 8 studies) was significantly higher than other treatments. Evidence was limited for laser treatment (1 study) and for topical and systemic treatments (mostly single case reports or small non-comparative series with limited follow-up).Conclusions Many observational studies have looked at different treatment modalities for SCC, but the evidence base for the effectiveness of these interventions is poor. Comparison of outcomes after different treatments should be interpreted cautiously owing to biases inherent in the types of study included, and lack of direct comparisons to enable the estimation of relative treatment effect. Further evidence is needed to develop a prognostic model and stratify individuals at high risk of developing SCC, to improve the evidence base for this common cancer and to optimise clinical management.Protocol registration International Prospective Register of Systematic Reviews (PROSPERO) registration number CRD42011001450.

Cilt tümörlerinin çoğunluğunu bazal hücreli ve yassı hücreli karsinomlar oluştururlar. Aurikula bölgesindeki tümörler bölgesel fleplerle yada cilt greftleri ile onarılabilirler. Bu yayında sol aurikulasındaki sentral yerleşimli 12 mm'lik cilt lezyonuna yassı hücreli karsinom tanısı konan hastanın yapılan eksizyonu sonrasındaki 2 cm lik defektinin postauriküler random transpozisyon flebi ile rekonstrüksiyonu sunulmuştur. AbstractSkin cancers are the most commonly seen tumors in human, due to sun exposure %80 of them are identified in head and neck region. Basal cell carcinoma and squamous cell carcinoma compose the majority of the skin tumors. The defects of the auricles can be reconstructed with local skin flaps or free skin grafts. In this report we represent a transpositional random flap reconstruction for a 2 cm central auricular defect after a 12 mm squamous cell carcinoma excision from left central auricule. Keywords GirişDeri kanserleri insanlardaki en sık tümöral oluşumlar-dır. Biyolojik davranışları oldukça farklı olan bu tümörler içinde bazal hücreli karsinom vakaların büyük bir kısmını oluşturur. Sıklık olarak sırasıyla yassı epitel hücreli karsinom, malign melanom ve deri eki tümörleri gelir 1 . Aurikulada skuamöz hücreli cilt kanseri oluşma sıklığı ise tüm baş boyun cilt tümörlerinin yaklaşık % 3-4 dür. Ancak baş boyun bölgesindeki skuamöz hücreli cilt kanserlerinin en kötü prognozlu olanları genelde aurikulada ve dudaklarda oluşanlardır [2][3][4] . Tanısı biopsi ile konulan deri kanserlerinin tedavisinde cerrahi rezeksiyon temel prensip olmasının yanısıra, elektrodesikkasyon ve küretaj, Mohs mikrocerrahisi, radyoterapi, kyrocerrahi, laser tedavisi, fotodinamik tedavi, 5-Flourourasil ve İmikuimod uygulanabilir [5][6][7][8] Vaka TakdimiÇiftçilikle uğraşan 43 yaşındaki erkek hastanın uzun süre-li güneş maruziyeti neticesinde değişik bölgelerinde oluşan multiple cilt kanserleri hikayesi mevcuttu. Objectives. The reconstruction after nasal skin cancer (NSC) resection is often practiced differently. The objective of this study is to evaluate the influence of patient-, tumor- and management-related factors on the role of surgery and choice of reconstruction.Methods. This was a monocentric retrospective study of patients who were diagnosed with a NSC (squamous cell or basal cell carcinoma) and suffered from an extended defect after ablative surgery between 2003 and 2013. Twenty-five patients were included. Tumors were staged using the Union for International Cancer Control (eighth edition) TNM classification for primary cutaneous skin cancer of the head and neck. Preferred treatment was surgery in all patients. Health-related quality of life (HRQoL) measurement was evaluated by one generic (36-Item Short Form Health Survey [SF-36]) and two organ-specific questionnaires (Rhinoplasty Outcome Evaluation [ROE] and Functional Rhinoplasty Outcome Inventory 17 [FROI-17]) after therapy. Survival data were estimated by the Kaplan-Meier method and statistical analysis was performed by log-rank, analysis of variance, Levene’s and t-tests. The median follow-up time was 2.1 years.Results. According to the Union for International Cancer Control classification, 13 of 25 tumors were staged as pT1 (52%), four as pT2 (16%), seven as pT3 (28%) and one as pT4a (4%). Seventy-two percent of patients (n=18) chose plastic reconstruction, and for the remaining 28% (n=7) of the patients opted for an implant-retained prosthesis. The overall survival was 69.5% after 5 years, the 5-year recurrence-free survival was 90.9% and the 5-year disease-specific survival was 100%. There was no significant difference in the HRQoL outcome between both rehabilitation methods.Conclusion. Surgery in NSC gives an excellent oncologic prognosis. Nasal reconstruction and prostheses are both very viable options depending on tumor stage and biology, the patient’s wishes as well as the experience of the surgeon.
