BackgroundVisual impairment (VI) affects physical, psychological, and emotional well-being, and social life as well. The purpose of this exploratory study was to assess the psycho-social impact of VI on health-related quality of life (HRQoL) among nursing home residents.MethodsThis cross-sectional study involved 272 residents of 60 years or older residing in seven nursing homes of the Kathmandu Valley, Nepal. Comprehensive ocular examinations, including near and distance vision assessment and refractions were carried out. VI was defined as visual acuity (VA) less than 6/18 in the better eye. Residents were divided into two groups: one group did not have VI (in whom VA was greater than or equal to 6/18 in the better eye), and the other had VI (in whom VA was worse than 6/18 in the better eye).Face-to-face interviews were conducted filling out a 36-item The Medical Outcomes Study Short-Form (SF-36) questionnaire. The SF-36 questionnaire was scored according to the scoring algorithm SF-36 subscales.ResultsThe mean age of residents was 74.68 ± 8.19 years (range, 60–99 years) and the majority were female (78.68%). The mean composite score of SF-36 was 46.98 ± 13.08. VI detrimentally affected scores of both the physical and the mental components, but the impact of VI was slightly greater for the physical component than that for the mental component. There was a trend towards a lower composite score as well as each subscale score of the SF-36 in participants with VI than in those without VI.ConclusionVI has a negative effect on HRQoL. HRQoL is reduced among nursing home residents and the reduction in the HRQoL bears a positive association with VI.

ZusammenfassungIn rehabilitationswissenschaftlichen Anwendungsgebieten werden wichtige Entscheidungen aufgrund von Beurteilungen und Einschätzungen durch z. B. Rehabilitanden, Therapeuten oder Peer-Reviewer getroffen. Beurteilungen desselben Sachverhalts (z. B. Qualität von Kommunikationsprozessen, Versorgungsqualität) variieren jedoch sowohl systematisch als auch zufallsbedingt: Merkmalseinschätzungen können z. B. davon abhängen, wer (einzelner Beurteiler) aus welcher Perspektive (z. B. Behandler- vs. Rehabilitandenperspektive) in welcher Situation zu welchem Messzeitpunkt welchen konkreten Aspekt beurteilt. In diesem Sinne spiegeln sich mehrere systematische Teilkomponenten in Beurteilungsdaten wider. Mittels der Generalisierbarkeitstheorie können sich überlagernde Informationskomponenten getrennt und hinsichtlich ihrer Bedeutsamkeit für die Beurteilungsergebnisse identifiziert werden. Hierdurch entsteht zum einen ein differenziertes Bild, welche Informationsanteile für die angemessene Interpretation von Beurteilungsdaten berücksichtigt werden müssen (Generalisierbarkeitsaspekt). Zum anderen kann begründet werden, wie Beurteilungssituationen gestalten werden sollten, damit die interessierenden Teilinformationen zuverlässig bestimmt werden können (Entscheidungsaspekt). In diesem Beitrag wird für typische Anwendungen in den Rehabilitationswissenschaften gezeigt, wie die Generalisierbarkeitstheorie zum besseren Verständnis und zur Optimierung von Beurteilungsdaten genutzt werden kann.

For diagnostics and outcome measurement in clinical rehabilitation a multitude of questionnaires is used. In order to gain comparability of the diagnostic findings, generally, the same information is gathered of all patients, regardless of their state of health or how severely ill they are, by using identical groups of items. In this kind of assessment it is, however, problematic that (a) the assessment instrument usually only allows for adequate and reliable diagnostics of patients who suffer from injuries or illnesses of medium severity, and (b) that an uneconomic data collection is required because of an extensive set of items which may in individual cases be uninformative and unnecessary. This article shows how data assessment can be adjusted to the individual handicaps of the patient by using adaptive or tailored testing. Thus it can be ensured that the testing becomes more economical and results more precise. Furthermore, a high acceptance by the patients participating in the data collection can be achieved, as the test items submitted fit the ability of the patients adequately. Yet, adaptive test systems require a complex and very careful psychometrical development of the assessment procedure in such a way that in spite of presenting different items to patients, diagnostic outcomes are comparable between patients and for different points in time (e. g., pre-post comparison). This article shows the steps of development that have to be accomplished in order to set up a psychometrically sound adaptive test procedure. Moreover, it shows how its usage in the area of medical rehabilitation can be profitable for handlers and patients.
