To conduct a three-stage data envelopment analysis (DEA) of the operational efficiency of urban employee basic medical insurance (UEBMI) across 31 provinces in China from 2012 to 2021. Methods: Utilized a three-stage DEA model, this study measured and evaluated the operational efficiency of urban employee basic medical insurance within China's 31 provinces. Results:The operational efficiency of urban employee basic medical insurance in China was notably low, displaying significant disparities across different regions and periods. Substantial room for improvement exists. Environmental factors, including urbanization level and unemployment rate, wielded a pronounced influence on the operational efficiency of China's employee medical insurance. Conversely, the fiscal revenue and expenditure ratio hindered the enhancement of employee medical insurance efficiency. Conclusion:This study held valuable insights for enhancing the operational efficiency of China's urban employee basic medical insurance. To effectively improve insurance efficiency, the following recommendations are put forth: firstly, relevant departments should escalate resource investments and optimize resource utilization based on rational allocation; secondly, enhance legislation and regulations, fortify fund oversight, and ensure equitable and judicious utilization of the medical insurance fund; finally, leveraging modern high-tech advancements can comprehensively elevate the operational efficiency of the employee medical insurance fund. Ultimately, with the advent of cutting-edge technology, the operational efficiency of employee medical insurance fund can be comprehensively elevated.

In the context of China’s comprehensive poverty alleviation efforts, this study explores the differences in the re-poverty risk between households that have been lifted out of poverty before and after policy withdrawal, as well as the sensitivity of different family types to their livelihood capital. The study used data from 45,141 out-of-poverty households in Yucheng County, Henan Province, from 2016 to 2020, and combined the poverty vulnerability theory and short-fall risk method to evaluate the re-poverty risk. The Tobit model was used to explore the influence of livelihood capital on the re-poverty risk. The study found that the overall re-poverty risk is 1.13%, which increases to 18.09% after direct poverty alleviation policy is withdrawn. The risk of working families is significantly lower than farming families. All kinds of livelihood capital significantly reduce the re-poverty risk, with natural capital playing the most significant role. For different family types, the marginal contribution of financial capital to reducing the re-poverty risk is relatively larger in working households, while that of natural capital is larger in farming households. Specifically, labor capacity, arable land area, local leaders, and loans have a more significant inhibitory effect on the re-poverty risk. These findings provide valuable insights for formulating policies related to increasing household income and preventing the occurrence of re-poverty.

BackgroundThere are many kinds of chronic diseases, high incidence and high hospitalization rate in older people caused by population aging. The increasing demand for healthcare services has become an increasingly prominent problem in Chinese society. The purpose of this paper is to explore the influence of social support on multiple healthcare services for older people and the urban-rural differences among them.MethodsThe data are from our field survey in Shaanxi province in 2019. Using the Anderson model as the basic explanatory framework, this paper uses the Tobit-model to analyses the impact of social support for older people on the utilization of therapeutic healthcare services, and the Logit-model to analyze the impact of living arrangements and social support on the utilization of preventive healthcare services for older people.ResultsThis paper examines the impact of formal and informal support on outpatient, inpatient, and preventive healthcare services for older people, provides an in-depth analysis of the differences in the impact of social support on healthcare service utilization between urban and rural older people and analyses the contribution of various factors to the impact. The coefficient effect is divided into two parts: the coefficient “premium” of urban older people relative to urban and rural older people as a whole, which accounts for 10.8% of the total difference; and the “premium” of urban and rural older people as a whole to rural older people, which accounts for 18.9% of the total difference. The coefficient effect accounts for 29.7 per cent of the total difference.ConclusionsRural older people place greater importance on the quantity of formal social support, while urban older people place greater importance on the quality of formal social support. The phenomenon of raising children for old age was evident in the use of healthcare services by rural older people, while the phenomenon of distant relatives being better than close neighbors was evident in the use of healthcare services by urban older people. Free preventive healthcare services in rural China have largely compensated for the lack of health benefits for rural older people.
