ResumoObjetivo: a hiperdrenagem devido a acidentes e manipulação inadequada de sistemas de drenagem externa é um fato e submete o paciente a conseqüências graves, muitas vezes até à morte. Essa complicação mecânica tem sido subestimada e para a qual não existe mecanismo protetor até esse momento. Este trabalho procura responder às seguintes questões: (1) qual é o tempo necessário para que ocorra a drenagem de todo o líquor contido nos ventrículos para o sistema de drenagem externa? (2) qual é a influência do gradiente hidrostático no tempo de drenagem? (3) qual a influência da pressão intraventricular no tempo de drenagem? Materiais e métodos: utilizando-se de uma bancada de testes para hidrodinâmica, as tubagens de três sistemas, comerciais disponíveis no país, foram testadas quanto às suas características de fluxo. Foram simulados gradientes hidrostáticos negativos de 12,5; 25; 50 e 100 cm, e pressão intraventricular (PIV) de 5, 10, 15 e 20 cm H2O. Resultados: o tempo de escoamento do líquor ventricular depende do raio e do comprimento do tubo do sistema utilizado. Entretanto, qualquer que seja o sistema utilizado, o escoamento ocorre muito rapidamente. O gradiente hidrostático é o fator mais importante na drenagem do conteúdo ventricular no efeito sifão; a PIV e o raio do tubo têm importâncias relativas, que se atenuam conforme aumenta o gradiente hidrostático negativo. Conclusão: a quantificação temporal do fluxo livre de uma bolsa de drenagem ressalta a importância clínica de se desenvolver mecanismos protetores contra o efeito sifão em sistemas de drenagem externa.

Part II: the siphoning effect on external ventricular drainage Objective: siphoning due to accidents and erroneous manipulation of external drainage systems is a fact, and may cause severe damage to the patient. This mechanical complication has been underestimated, and do not exist a preventive mechanisms against it. In this study, we submitted three commercial external drainage systems to a bench test, in order to evaluate the flow carachteristics and siphoning effect of them under different simulations of intracranial pressure and negative hidrostatic gradient. We have tried to answer the following questions: 1) what is the time necessary to ocurr the drainage of a volume of fluid similar to the ventricular content? 2) what is the influence of the hydrostatic gradient in the siphoning effect? 3) what is the influence of intracranial pressure in the siphoning effect? Biodegradable Polymer Releasing Antibiotic Developed for Drainage Catheter of Cerebrospinal Fluid: In Vitro Results

The authors developed a biodegradable polymer that releases an antibiotic (nalidixic acid) slowly and continuously, for prevention of catheter-induced infection during drainage of cerebrospinal fluid. We investigated the in vitro antibiotic releasing characteristics and bacterial killing effects of the new polymer against E. coli. The novel fluoroquinolone polymer was prepared using diisopropylcarbodiimide, poly (e-caprolactone) diol, and nalidixic acid. FT-IR, mass spectrometry, and elemental analysis proved that the novel antibacterial polymer was prepared successfully without any side products. Negative MS showed that the released drug has a similar molecular weight (M.W.=232, 350) to pure drug (M.W.=232). In high pressure liquid chromatography, the released drug and drug-oligomer showed similar retention times (about 4.5-5 min) in comparison to pure drug (4.5 min). The released nalidixic acid and nalidixic acid derivatives have antibacterial characteristics against E. coli, Staphylococcus aureus, and Salmonella typhi, of more than 3 months duration. This study suggests the possibility of applying this new polymer to manufacture drainage catheters that resist catheter-induced infection, by delivering antibiotics for a longer period of more than 1 month.

Efficacy of Vancomycin/Tri-iododecyclemethyl Ammonium Chloride-coated Ventriculostomy Catheters in Reducing Infection

Abstract: Vancomycin-bonded, TDMAC-coated ventriculostomy catheters bind and release the drug at levels exceeding the minimum inhibitory concentration for S. aureus and S. epidermidis for at least 6 days and can significantly delay the onset of infectious ventriculitis in a rabbit model.
