Objectives: This study was conducted to assess the outcome of the use of the Accent method of voice therapy as an initial approach for patients with benign vocal fold lesions. Patients and Methods: This experimental prospective study was conducted on 30 patients (10 females and 20 males), aged 18 -60 years (mean age: 40.6) with benign vocal fold lesions. Auditory perceptual analysis, videolaryngoscopic examination of the larynx, acoustic analysis and Voice handicap index (VHI) were performed for all patients before and after voice therapy. Results: Auditory perceptual assessment and voice handicap index (VHI) done for patients with benign vocal fold lesions before and after voice therapy revealed significant improvement (p value 0.003: p value 0.000 respectively). The acoustic parameters for the different group of benign vocal fold lesions pre-therapy in comparison to the acoustic values post voice therapy revealed significant improvement in the fundamental frequency (p value >0.001). Videostroboscopic examination revealed complete recovery in 6 cases of vocal fold nodules, mild improvement of 5 cases of vocal fold polyp with complete cure of one case. There was mild improvement of all cases of vocal fold cysts, contact granuloma and Reinke's edema without complete cure except for complete cure of two cases of Reinke's edema. Conclusion:The Accent method of voice therapy may lead to an improvement in the perceived voice quality in some patients with benign vocal fold lesions, and this could make surgical intervention unnecessary.

IntroductionSpeech production includes segmental and suprasegmental features, which interact and cooperate with each other. Impaired speech production is common in individuals with post-stroke dysarthria. The commonly used phonemic contrast therapy and Accent Method in clinical practice can improve the segmental and suprasegmental aspects, respectively. This study aimed to explore the feasibility and immediate effectiveness of the combination of specific rhythm patterns of the Accent Method and phonemic contrast on speech production.MethodsFifteen poststroke dysarthria patients (12 males and three females) first received accentuation task of three rhythm patterns (Largo-slow, Andante-medium, Allegro-fast) and later received speech task in appropriate rhythm patterns combined with phonemic contrast materials and non-phonemic contrast materials. Speech parameters were analyzed by considering speech clarity and prosody.ResultsThe results showed that the number of correct target syllables, sentence clarity, and standard deviation of intensity increased significantly, and the average length of pauses and abnormal pause times decreased significantly in Andante (medium) compared to other rhythms. The number of correct target syllables, sentence clarity, and standard deviation of intensity increased significantly compared with those in the non-phonemic contrast in Andante (medium).ConclusionThe combination of phonemic contrast and Accent Method was verified to have an immediate effect on speech production in Mandarin speakers with post-stroke dysarthria and could be further validated in other diseases with impaired speech production in the clinic in the future.

Background This work aims to compare between stretch-and-flow voice therapy and Smith accent method of voice therapy in treatment of hyperfunctional dysphonia in order to assess the benefit of using this new technique in its management. A randomized controlled trial was conducted at the Unit of Phoniatrics, Otorhinolaryngology Department, in the period from November 2020 to February 2023. A sample of 60 patients with hyperfunctional dysphonia was randomly divided into two groups; each group consisted of 30 patients: “group A” who received stretch-and-flow voice therapy (SnF) and “group B” who received Smith accent (SA) method of voice therapy. Assessment included auditory perceptual assessment (APA) using modified GRBAS scale and Arabic-voice handicap index (Arabic-VHI). Results The study revealed significant improvement in grade of dysphonia, voice quality, and Arabic-VHI post-therapy for both groups. There was a significant improvement regarding voice pitch in SnF group only and in loudness of voice in SA group only. Conclusion SnF can be considered as an alternative approach for treating hyperfunctional dysphonia, offering a shorter duration, and potentially being a simpler alternative for patients who struggle with the more intricate rhythms of the Smith Aaccent voice therapy method.
