Efficacy of surgical microwave ablation for recurrent hepatocellular carcinoma after curative hepatectomy

Abstract: Background: Little evidence exists regarding postrecurrence survival after microwave ablation for recurrent hepatocellular carcinoma (HCC) after curative hepatectomy; we aimed to evaluate the feasibility of surgical microwave ablation. Methods: In this retrospective review, we enrolled patients who underwent curative hepatectomy for primary HCC in our department and had intrahepatic recurrence. We analyzed overall survival according to treatment modality to clarify the prognostic factors for survival. Results: O… Show more

We have performed surgical microwave ablation, termed microwave coagulo-necrotic therapy (MCN), as one of the treatment modalities of HCC patients for more than 20 years, and previously reported its feasibility, safety, and long-term outcomes [14][15][16][17] . In our recent propensity score matching analysis, we clarified that MCN provided almost equivalent OS and RFS as hepatic resection for single HCC of < 5 cm with less invasive perioperative outcomes 15 .…”

“…Several previous studies have also reported that microwave ablation showed comparative results compared to RFA for primary HCC when comparing local recurrence rates and long-term survival with 5-year OS of 43–60% 11 14 , 27 . “…36 Thus, PLR combined with ablation or transarterial chemoembolization (TACE) may be a preferred program for some HCC patients with poor surgical tolerance or advanced tumor stage. 37,38 In this context, CTC may help to select candidates for PLR vs LT.…”

“…1,2 Due to the inconspicuous early symptoms, many HCC patients are only found to be in the advanced stage once diagnosed, depriving them of the opportunity for surgery and treatment. 3 Currently, hepatic artery perfusion chemotherapy is the mainstream treatment method for HCC; however, whose efficacy and prognosis turn out to be poor due to the emergence of resistance of HCC cells. 4 Therefore, exploring the mechanism of drug resistance of HCC cells is of great clinical significance for the treatment of HCC.…”
