Background and Aim: Pulmonary hypertension (PH) is a common complication of degenerative mitral valve disease (DMVD). Sildenafil, a phosphodiesterase-5 inhibitor, has effects in reducing pulmonary arterial pressure by selectively dilating pulmonary vessels. The study aimed to evaluate the effects of sildenafil in combination with conventional therapy in dogs with PH caused by DMVD. Materials and Methods: Fourteen dogs diagnosed with PH secondary to DMVD Stage C on conventional therapy were randomly assigned to placebo (n=7) and sildenafil (n=7) groups. On day 0, the recruited dogs underwent physical examinations, clinical score assessments, electrocardiography, systolic blood pressure measurements, blood collection, thoracic radiography, and echocardiography for baseline. The dogs then received a combination of conventional therapy with sildenafil or placebo every 8 h for 1 week. On day 7, all dogs underwent the baseline evaluations again. Results: The sildenafil group experienced a significant decrease in estimated systolic pulmonary artery pressure (sPAP) (p=0.043) from day 0 to day 7. Moreover, the total clinical scores were decreased in dogs treated with sildenafil relative to those who received the placebo (p=0.007); however, the lung scores were not different between before and after treatment with sildenafil. Conclusion: Sildenafil had a synergistic effect with conventional therapy in reducing the estimated sPAP and clinical scores in dogs with PH secondary to DMVD.

OBJECTIVE To develop a topical sildenafil hydrogel and evaluate its effect on wound healing in dogs. ANIMALS 6 purpose-bred, sexually intact, adult Beagles. PROCEDURES Hydrogels containing sildenafil citrate, N-methyl-2-pyrrolidone, propylene glycol, and poloxamer 407 were developed. Four excision wounds were created along the dorsum of the dogs. Each wound was treated for 21 days with a nonadherent bandage (C) or with a hydrogel containing 0% (G), 5% (5S), or 10% (10S) sildenafil. Daily bandage changes with wound imaging were performed. Biopsy specimens were collected 5 times. RESULTS Hydrogels were homogenous at room temperature and released > 90% of the sildenafil within 8 hours in vitro. Time to first granulation tissue was significantly shorter for the sildenafil groups (mean ± SD, 2.8 ± 0.8 days [5S and 10S]), compared with the control groups (5.2 ± 0.4 days [C] and 6.3 ± 1.4 days [G]). The G wounds had a 10% to 14% lower contraction rate, compared with the C, 5S, and 10S wounds. 5S wounds had a total wound area 0.7 ± 0.3 cm2 larger than 10S wounds. No significant differences were present when C wounds were compared with 5S and 10S wounds for total wound area, contraction, or epithelialization. Histologic acute inflammatory scores were higher for 5S and 10S wounds in the early and late stages of wound healing, with higher reparative scores on day 7. Neovascularization was higher for 10S wounds on day 7 and 14. CLINICAL RELEVANCE The topical sildenafil hydrogel promoted early granulation tissue, which may be beneficial for secondary wound closure in clinical settings.

Degenerative mitral valve disease (DMVD) is the most common acquired cardiac disease in small breed dogs. Pulmonary hypertension (PH) is a common complication in DMVD that can worsen the clinical signs, cardiac function, and median survival time of DMVD dogs. Conventional therapy of DMVD can reduce systolic pulmonary arterial pressure (sPAP) minimally. Thus, additional therapy is required. Sildenafil is used to treatment of PH in various causes, but the effect of sildenafil in the treatment of PH secondary to DMVD is not published yet. Therefore, this study aimed to evaluate the effects of sildenafil in combination with conventional therapy in PH dogs caused by DMVD. Fourteen dogs were diagnosed with PH secondary to DMVD stage C and were being on conventional therapy. The recruited dogs were randomly assigned to the placebo (n=7) and sildenafil (n=7) groups. At day 0, the recruited dogs were performed a physical examination, clinical scores assessment, electrocardiography (ECG) measurement, systolic blood pressure measurement (SBP), blood collection, thoracic radiography examination, and echocardiography examination for baseline. Dogs in the sildenafil group received a combination of conventional therapy and sildenafil. The median dose of sildenafil was 1.79 (1.69-2.19) mg/kg every 8 hours for 1 week, while the placebo group received a placebo with the same regimen. At day 7, the recruited dogs were performed all examinations same as day 0. The results showed that the sildenafil group had a significant decrease in sPAP (p=0.043), while the sPAP did not change in the placebo group. However, clinical and lung scores did not improve after treatment with sildenafil. Additionally, this study found that sPAP had a positive correlation with heart rate and isovolumetric contraction time of right ventricle (p=0.005 and 0.014, respectively). In conclusion, sildenafil had a synergist effect to conventional therapy in reducing sPAP but effects of sildenafil on clinical and lung scores were not found.
