Efficacy of sildenafil in erectile dysfunction after radical prostatectomy

Abstract: Radical retropubic prostatectomy (RRP) is an important cause of iatrogenic erectile dysfunction (ED). While sildena®l has been widely used since its introduction as a new treatment option for ED, its ef®cacy in post-RRP patients has not been extensively studied. We retrospectively compared the ef®cacy of sildena®l in post-RRP and non-surgical patients with ED (NSED) using a subset of questions from the International Index of Erectile Function (IIEF) and correlated results with their speci®c etiology of ED base… Show more

“…Mean scores were 1.0 for those with unilateral surgery (n ¼ 12) and 1.5 for those who IIEF: a review RC Rosen et al had non-nerve sparing surgery (n ¼ 26). Similar findings were obtained in studies by Feng et al, 40 Blander et al 41 and Jarow. 42 In each of these studies, scores on the IIEF were noticeably and predictably lower for the prostatectomy patients relative to ED patients with mixed etiologies in the earlier multicenter trials.…”

“…In PPED, responsiveness to acute oral PDE5i resulted variable, but overall lower than in patients with other forms of ED. [39][40][41][42] The present results suggest that PDE5i administration should be started as soon as possible after surgery.…”

“…After radical prostatectomy, 29 -53% of men report a positive response. [2][3][4][5][6] Men who are younger, who undergo bilateral nerve-sparing surgery, and ware are at least 1 year postsurgery are more likely to fall into this group. [3][4][5][6] The positive response rate to sildenafil after radiation therapy for localized prostate carcinoma varies from 50% to 91%, with higher success rates reported among men who received brachytherapy.…”

“…Case series that used the ability to have intercourse consistently over several weeks as the outcome criterion reported much lower success rates compared with other series. 2,5,6,8,10,11 In general, sildenafil is most effective in men who have psychogenic ED or only a mild, organic deficit in penile rigidity. 12,13 Its efficacy also clearly is better in men who have a greater degree of recovery of tumescence after treatment for prostate carcinoma.…”
