Background -Otodectes cynotis (ear mite) accounts for 50-80% of cases of otitis externa in cats. Various treatment options exist but evidence-based reviews on treatments for this parasite have not been conducted.Hypothesis/Objectives -The purpose of this study was to systematically review the efficacy of treatments for Otodectes cynotis infestation in cats, to provide recommendations for their use and to suggest methodologies for future clinical trials and research.Methods -Electronic searches were carried out using the PubMed, CAB Direct and Thomson Reuters Web of Science; relevant topics were hand searched. The review was restricted to peer reviewed articles without limitation of language and publication date; only in vivo studies were included. The data were extracted and tabulated, then compared with regard to study design. With only a few randomized and controlled trials (RCTs) available, it was not possible to perform a meta-analysis of the pooled data.Results -A total of 27 trials published between 1978 and 2015 fulfilled the selection criteria; 17 different pharmacological interventions were identified. There were two blinded RCTs and 25 open uncontrolled trials.Conclusions and clinical importance -There is fair evidence for recommending spot-on 10% imidacloprid +1% moxidectin or selamectin, once or twice 30 days apart, while there is insufficient evidence for or against recommending all other medications due to the quality of study designs in spite of their high efficacies. Future blinded RCTs, using placebo or appropriate active drugs, are required; outcome measurements on both clinical and microbiological and parasitological improvement, and proper follow-up periods are recommended.

Assessment of the Ear Mite (Otodectes cynotis) Infestation and the Efficacy of an Imidacloprid plus Moxidectin Combination in the Treatment of Otoacariosis in a Hungarian Cat Shelter

Two separated field studies were carried out in Hungary to assess Otodectes cynotis infestation and to evaluate the efficacy of 10 % imidacloprid and 1.0 % moxidectin spot-on for cats against ear mites in a cat shelter. Mite infestation was studied with direct, otoscopic examination of the external ear canals and with the presence or absence of live mites in samples taken with cotton-tipped swabs from both ears of each animal. Fifteen out of 100 cats selected randomly were found to be infested. Seven animals showed severe clinical signs of otitis externa consistent with O. cynotis infestation. These cats were restless, shaking their heads and scratching their ears. Alopecia, wounds caused by self-injuries and dermatitis were seen in and around the ears of the two cats having most severe clinical signs. The other infested cats did not show any clinical signs, only dark brown exudates ranging from moist and waxy to crumbly and flaky accumulated at the deeper part of the external ear canals.Ten cats were randomly assigned to the treatment group and five animals to the untreated control group. Each cat of the treated group was weighed and treated with the recommended dosage of 10 % imidacloprid and 1.0% moxidectin spot-on on day 0. Only those three cats were found to be infested with live ear mites on day 9, which had showed the most severe clinical signs of otitis externa before treatment. The black-brown cerumen or scab was drier in their ear canals than nine days earlier. Some live and dead mites were found only by microscopic examination of aural debris. The efficacy of 10 % imidacloprid and 1.0% moxidectin against O. cynotis was 100 % on days 16 and 30. Direct and otoscopic examinations of both ears of treated animals could not reveal mite movement. A very small amount of S35 dry material was found in the ear canals of the formerly most affected cats. No or some dried up mites occurred in samples taken with cotton-tipped swabs from all cats which had no clinical signs of otoacariosis. The external ear canals of untreated cats continued to contain large amounts of dark brown, waxy cerumen and their ear swab samples contained live ear mites until the end of study.

A study based on naturally infested cats was designed to evaluate the effectiveness of a single treatment with a topical formulation containing fipronil, (S)-methoprene, eprinomectin and praziquantel, for the prevention of Otodectes cynotis infestation in cats. Six treated cats and six untreated cats were housed with three chronically Otodectes cynotis-infested cats, respectively. The cats of each group were kept together in a 20-m2 room for 1 month. Both clinical examination and ear mite counts were conducted on Day 28. All donor cats were confirmed to be chronically infested with Otodectes cynotis on Day −1 and Day 28. From untreated control cats, 129 live mites were recovered on Day 28 and all cats were found to be infested. In the treated group, three cats were found to be infested, with a total of five live mites recovered, the difference between the two groups being significant (p = 0.003). One treatment corresponded to 96% preventive efficacy at Day 28 based on ear mite counts. With regard to cerumen, the clinical score increased significantly for untreated cats between Day −1 and Day 28 (p = 0.00026) and not for treated cats (p = 0.30). The difference in cerumen abundance was significant between untreated and treated cats on Day 28 (p = 0.0035). Concerning the pruritic reflex in at least one ear, all cats were negative at inclusion. All six untreated cats became positive and showed a reflex on Day 28, whereas no treated cat showed ear pruritus (p = 0.00026).
