Efficacy of Iodopovidone Pleurodesis and Comparison of Small-Bore Catheter Versus Large-Bore Chest Tube

Abstract: Iodopovidone is an effective, inexpensive, safe, and easily available alternative in chemical pleurodesis in MPE. The success rates of pleurodesis were found to be similar regardless of the type of the tube inserted.

“…More recently, a study that included 43 patients with malign pleural effusions showed a success rate of 88%, and small bore catheters and large bore chest tubes were used to administer the iodopovidone into the pleural space in these cases. [13] Agarwal et al [10] reported comparable results in 37 patients who received iodopovidone through a chest tube. There were no reports of any recurrences of the effusions in 32 (86.5%) of their patients.…”

“…Forty-one patients developed hypertensive peaks after iodopovidone instillation, including 10 serious events, and 4 patients developed hypotension, including 1 serious event. Hypotension has already been reported after iodopovidone pleurodesis [15,16,17]; however, in previous studies, hypertensive peaks have not been mentioned. In our study, many of these episodes were not associated with pain; therefore, we believe that most hypertensive peaks were due to procedure-related anxiety.…”

“…It occurred in 33 patients (55%) and was classified as serious in 11 of them (18%). Pain had already been reported as a complication of pleurodesis with iodopovidone as well as with other sclerosing agents [12,13,14,15,16,17,18,19]. Agarwal et al [16] found a median VAS score of 50.5 (range 10-95) after iodopovidone pleurodesis through the chest tube in patients with MPE.…”
